Article 46 Medical and health personnel shall truly, accurately and completely record vaccination information such as vaccine varieties, identity information of marketing license holders, minimum packaging unit, expiration date, vaccination time, medical and health personnel who carried out vaccination, and recipients. In accordance with the provisions of the health authorities in the State Council, ensure that vaccination information can be traced back and inquired. Vaccination records shall be kept for not less than five years after the vaccine expires.
Article 47 The State practices a system of children's vaccination certificates. Within one month after a child is born, his guardian shall apply for a vaccination certificate at the vaccination unit or the birth hospital where the child lives. The vaccination unit or the hospital of birth shall not refuse to handle it. The guardian shall properly keep the vaccination certificate.
Vaccination is managed by the place of residence. During the period when children leave their original residence, the vaccination unit that undertakes vaccination at their current residence is responsible for vaccination.
The format of vaccination certificate shall be stipulated by the competent health department of the State Council.
