Required materials: When handling the certificate, bring my household registration book (it is best for both husband and wife, and if the household registration is not together, bring my mother's household registration book), ID cards of both parties, and the certificate of first marriage and first childbirth of both husband and wife (which can be issued by the work unit or the neighborhood Committee), and go to the street office to handle the birth permit.
Other required materials:
A. medical insurance manual and its copy;
B pregnancy diagnosis certificate and pregnancy test sheet issued by the designated hospital (stamped with birth seal);
C 1 inch bareheaded photo of the woman.
