According to the Notice of the General Office of Hunan Provincial People's Government on Launching the Pilot Work of Residents' Health Card Application (Hunan Zhengban Fa [2065438+06] No.2) and the Notice of Hunan Provincial Health and Family Planning Commission on Launching the Information Adoption System of Residents' Health Card (Han [2065438+06] No.98), it is decided to launch the work of "Residents' Health Card" among on-the-job and retired employees in the school after research.
First, the residents' health card function
1. identification. Resident Health Card is an electronic information card which is uniformly planned and deployed by the National Health and Family Planning Commission and can be used by various health and family planning institutions at all levels in the country. It is the only proof that all residents will receive various types of continuous health services nationwide in the future. It has the functions of resident identification and personal basic health information storage.
2. Medical card. It can seek medical treatment across regions and institutions, and realize one-card health management for residents' medical treatment, fund settlement and the whole process.
3. Financial services. It has the function of bank debit card.
Second, the handling process
1. The applicant for the Resident Health Card shall truthfully fill in his/her relevant information (name, second-generation resident ID number, household registration address, residential address, his/her mobile phone number, contact person's name and contact person's mobile phone number) to the part-time family planning management personnel of the unit.
2. The family planning management personnel will summarize the information of the unit and send it to the Family Planning Office, Xiangya Campus and Railway Campus of the Ministry in the form of spreadsheets before May 20.
3. The school family planning management office summarizes the information and enters it into the health card database system. After the information is checked correctly, the bank will handle the card.
4. I hold a valid identity document to receive the "Resident Health Card".
