General hospitals above the town level can apply for health certificates. In fact, the main examination item is to test liver function to see if there is hepatitis B, and the others are to test chest radiograph, vision and blood pressure. It is valid for one year and can be searched online.
However, some work units specify which hospital to go to for physical examination, so I suggest that you don't rush to get a health certificate before you decide to work (provided you know you are healthy), and you have to have a blood test again.
Generally do a health certificate in 50 yuan or so. (ps: health certificate blood test only tells you whether you have hepatitis B, not whether you have antibodies. If you want a test sheet, you need to pay extra. )
