Fill in other information
Check "Yes" or "No" in the item "Whether to take commercial transportation" according to the actual situation. If "Yes" is selected, please fill in "Flight/Ship Location/Train Number" and "Seat Number". If "No" is selected, there is no need to fill it in. Then fill in the type and number of mobile phone number in turn, and the names and numbers of other effective contacts in China. Select the province, city and district from the drop-down list of "Domestic Residence".
Fill in the residence history
Fill in the living history of 14 days according to your actual situation. Select the start date, end date, country and region from the drop-down list.
Note: The residence history within 14 days must be completely covered, and the date must meet the requirements of 14 days, otherwise it cannot be submitted!
Fill in the symptoms
Fill in the physical symptoms truthfully according to your actual situation. If Yes is selected, check the corresponding symptoms; If No is selected, there is no need to check it.
Is the sixth step to enter Macao?
In "Are you an immigrant from Macau?" Check "Yes". Item 5 "COVID-19 nucleic acid test results 48 hours before your entry?" You don't have to fill it out.
