2. The employer fills in and submits the "Declaration Form" online;
3, safety supervision department for examination and filing;
4. After the online examination and approval, the employer shall print the "Declaration Form" in time for examination and filing, affix the official seal and sign it by the main person in charge of the unit, and submit it to the corresponding safety supervision department together with relevant documents and materials in accordance with the provisions of Articles 4 and 5 of the Measures for Declaration;
5. After receiving the paper application form submitted by the employer, the safety supervision department shall, within five working days, issue a receipt for the employer to declare the occupational hazard project, and classify the application form into the occupational hazard project management file.
To declare occupational hazards, the following materials shall be provided:
1, Application Form for Occupational Hazards Project signed by the main person in charge of the unit and stamped with the official seal of the unit;
2, the basic situation of the employer;
3. Types, distribution and telephone numbers of occupational hazard factors in the workplace.
To sum up, the employer gives priority to electronic data declaration through the "occupational disease hazard project declaration system", and the safety supervision bureau conducts on-site audit and online audit. After the examination and approval, the enterprise shall affix its official seal on the Application Form for Occupational Hazards Project and sign it by the principal responsible person of the employing unit, and submit it together with relevant documents and materials to the local city and county safety supervision administrative department for the record in accordance with the provisions of Articles 4 and 5 of these Measures.

Article 15 of the Law of People's Republic of China (PRC) on the Prevention and Control of Occupational Diseases
New construction, expansion and reconstruction projects, technological transformation and technology introduction projects (hereinafter referred to as construction projects) may produce occupational hazards, and the construction unit shall submit a pre-evaluation report on occupational hazards to the administrative department of health at the feasibility demonstration stage. The health administrative department shall make an audit decision within 30 days from the date of receiving the pre-evaluation report of occupational disease hazards, and notify the construction unit in writing. If the pre-evaluation report is not submitted or the pre-evaluation report is not approved by the health administrative department, the relevant departments shall not approve the construction project.
The pre-evaluation report on occupational disease hazards shall evaluate the occupational disease hazards that may occur in the construction project and their impact on the health of the workplace and workers, and determine the hazard categories and occupational disease protection measures.
Article 17
The pre-evaluation of occupational hazards and the evaluation of the control effect of occupational hazards shall be carried out by the occupational health technical service institutions established according to law and approved by the health administrative department of the people's government at or above the provincial level. The evaluation of occupational health technical service institutions should be objective and true.
