2. Please be on an empty stomach and bring two recent photos with the same background 1 inch. Medical examination fee 74 yuan/person.
3. After receiving the physical examination form, please fill in the date of physical examination and the first three lines of the physical examination form (personal information is confirmed and cannot be changed at will), and post photos at the designated place of physical examination.
4. Physical examination must be submitted to the doctor with 1 photos, and health training papers must be collected.
5. Please hand in the physical examination form after the intestinal examination. Those who fail to hand in the form cannot get a health certificate.
6. The examination contents mainly include: consultation, internal medicine, dermatology, blood sample examination, intestinal pathogenic bacteria examination, etc.
7. No photos, unqualified photos will not be reviewed. The physical examination conducted by others or others on their behalf shall be deemed invalid, and the physical examination form shall be invalid.
8. After the physical examination, seven days later, get the health certificate from 2:30-5:30 pm from Monday to Friday with the invoice and answer sheet.
