Urban counties should be in accordance with the interim management measures, the community health service work into the unified planning of community construction, strengthen leadership, supervision and management, pay attention to the typical, and create a new situation of community health service and community construction in our city.

Measures for the administration of urban community health service institutions in Hengshui (Provisional)
Article 1 In order to strengthen the standardized management of urban community health service institutions, improve the community health service system, and ensure the health of community residents, these measures are formulated according to the Regulations on the Administration of Medical Institutions in the State Council, the Guiding Opinions on the Reform of Urban Medical and Health System forwarded by the General Office of the State Council, and the Implementation Opinions on the Development of Urban Community Health Service in Hebei Province issued by ten departments including the Provincial Health Department, and combined with the actual situation of our city.
Second these Measures shall apply to the urban community health service institutions in our city.
Article 3 Urban community health service institutions are non-profit grass-roots health institutions that aim at solving the main health problems in the community and meeting the basic health service needs of residents, maintain and promote the health of urban community residents, and provide comprehensive services such as prevention, medical treatment, health care, rehabilitation, health education and family planning technical services.
Article 4 The municipal public health administrative department shall be responsible for the supervision and management of urban community health service institutions. The district health administrative department is responsible for the supervision and management of community health service institutions established by medical institutions under its jurisdiction. The community management department exercises the supervision, merger and construction rights of the community health service institutions within its jurisdiction.
Article 5 The construction of community health service institutions is an important part of community construction, and health service is an important content of community service. Community construction offices, sub-district offices and neighborhood committees at all levels should actively cooperate with and support health administrative departments and community health service institutions to do a good job in community health service, so as to make the community health service work develop smoothly and healthily.
Sixth community health service centers (stations) at all levels accept the leadership of the health administrative department at the same level, and accept the supervision and guidance of the community construction office, street offices and community neighborhood committees at the same level.
Article 7 The establishment of community health service institutions shall conform to the Planning for the Establishment of Urban Community Health Service Institutions in Hengshui City and the Basic Standards for the Establishment of Urban Community Health Service Institutions in Hebei Province.
Eighth community health service institutions set up examination and approval authority.
Municipal medical institutions and outpatient departments shall set up community health service institutions according to the plan, which shall be examined and approved by the municipal public health administrative department;
Medical institutions under the jurisdiction of the health administrative department of Taocheng District, namely, the Second People's Hospital of Hengshui City, the Fourth People's Hospital of Hengshui City, the Fifth People's Hospital of Hengshui City, the Maternal and Child Health Hospital of Taocheng District and individual clinics approved by the district health administrative department, set up community health service institutions for examination and approval according to the prescribed community scope, and the district health administrative department will conduct preliminary examination before reporting to the municipal health administrative department for examination and approval.
All approved community health service institutions shall be reported to the provincial health administrative department for the record and copied to the community construction office at the same level.
Ninth units or individuals to set up community health service institutions, must apply to the administrative department of health in accordance with the provisions of Article 8 of these measures. If the establishment is approved, it shall be issued a "Letter of Approval for the Establishment of Community Health Service Institutions".
Article 10 To apply for the establishment of a community health service institution, the following materials shall be submitted:
1. Application for establishment;
2. Feasibility report;
3, housing, equipment, personnel basic situation
4. The preliminary report of the district health administrative department (limited to the community health service institutions set up by medical institutions under the jurisdiction of the district health administrative department).
5, the relevant materials stipulated by the administrative department of health.
Eleventh community health service institutions must apply to the administrative department of health for practice registration and verification in accordance with the relevant provisions of the authority, and receive the "Practice License for Medical Institutions" and "Practice License for Community Health Service Institutions".
Twelfth community health service institutions are divided into community health service centers and community health service stations;
Community health service centers are generally located within the jurisdiction of sub-district offices, serving about 20,000-30,000 people;
As a supplement to community health service centers, community health service stations can be set up in areas that are difficult to be covered by community health service centers, and the service population should generally be around 5000.

(2) Targeted health guidance, behavioral intervention and screening of chronic non-communicable diseases, endemic diseases and parasitic diseases, as well as monitoring and standardized management of high-risk groups.
(3) Responsible for immunization and prevention and treatment of infectious diseases within the jurisdiction.
⑷ Provide appropriate Chinese and Western medicines and technologies, and carry out the diagnosis and treatment of common diseases and frequently-occurring diseases.

(6) provide family health care services, such as home visits, home care and home beds.
Provide counseling and referral services at one time.
(8) provide hospice care services.
(9) Providing mental health services and mental health consulting services.
⑽ Provide medical and health care services for key groups such as women, children, the elderly, patients with chronic diseases and the disabled.

(13) Carry out family planning consultation and publicity, and provide appropriate technical services.
14. Provide continuous health management services for individuals and families.
⒂ Responsible for the collection, collation, statistics, analysis and reporting of community health service information within the jurisdiction.
[14] In community construction, assist community construction departments to continuously expand community services, prosperity of the community culture, beautify the community environment, and create a healthy, progressive, civilized and harmonious community atmosphere.
⒄ Provide other appropriate primary health services according to the functions of community health services and the needs of community residents.
2. Infrastructure
(a) the use area of the business premises is more than 400 square meters, and the layout is reasonable, which meets the national hygiene standards and reflects the barrier-free design requirements.
⑵ Appropriate number of beds can be set according to the function of community health service, residents' needs and community resources.

3. Department setting: there are special places for general diagnosis and treatment, nursing, rehabilitation, health education, maternal and child health care, information management, etc.
4. Staffing
(1) Professional and technical personnel engaged in community health services must have legal qualifications.
(two) according to the needs of the function, task and service population, equipped with appropriate categories, levels and quantity of health technical personnel. There are at least 8 health technicians per 10,000 people in the jurisdiction, including 2 clinicians who have been trained by general practitioners and have intermediate titles or above, and 3-4 community nurses and preventive health care personnel.
Nursing and preventive health care personnel need to be trained in general medicine and related professional knowledge before taking up their posts.
5. Management system
(1) Professional ethics and code of conduct of all kinds of personnel in community health service institutions;

(3) Training, management, assessment and reward and punishment system for all kinds of personnel in community health service institutions;

(2) Setting standards of urban community health service stations
1, basic function
(1) Carry out community health survey and assist community management departments to implement health promotion;

(3) Carry out the diagnosis and treatment of common diseases and frequently-occurring diseases, and make clear the diagnosis of chronic diseases.

(seven) to provide health care services for key groups such as women, children, the elderly, patients with chronic diseases and the disabled;

⑾ Provide continuous health management services for individuals and families;
⑿ In community construction, assist community management departments to continuously expand community services, prosperity of the community culture, beautify the community environment, and create a healthy, progressive, civilized and harmonious community atmosphere;
According to the function of community health service and the needs of community residents, provide other suitable primary health services.
2. Basic facilities: the use area of the business premises is more than 60 square meters, the layout is reasonable, there are health education billboards, which meet the national health standards and reflect the barrier-free design requirements.
3. Department setting: There must be at least outpatient treatment room, treatment room, pharmacy and preventive health care room.

Thirteenth community health service personnel are mainly composed of general practitioners, community nurses, preventive health care and other related professional health technology and management personnel. Health technicians engaged in community health services must have legal qualifications, and can be temporarily replaced by clinicians trained by general practitioners before the national general practitioner qualification accreditation system is generally implemented. Nurses need to receive necessary community nursing training before taking up their posts.
Fourteenth community health service institutions must be unified and standardized.
The name of the community health service center is: county (city, district) name+street+logo name (optional)+community health service center;
The name of the community health service station is: the street where it is located+the name of the residential area where it is located+the community health service station.
Fifteenth to carry out community health services should focus on residents' health, take the family as the unit, take the community as the scope, take in-depth service for residents' families as the main form, strengthen health management, promote health promotion, and provide comprehensive health services for individuals and families.
Sixteenth community health service institutions should establish a two-way referral system with general hospitals, traditional Chinese medicine hospitals and specialized hospitals above the second level, accept the professional and technical guidance of preventive health care institutions, and improve the overall level of community health services.
Seventeenth community health service institutions must abide by the Law on Medical Practitioners, the Law on the Prevention and Control of Infectious Diseases, the Law on Maternal and Child Health Care, the Drug Administration Law and the Regulations on the Administration of Medical Institutions and other laws and regulations and related technical specifications.
Eighteenth community health service institutions must establish and improve various rules and regulations, standardize management and improve service quality in accordance with the requirements of the Basic Standards for the Establishment of Urban Community Health Service Institutions in Hebei Province.
Nineteenth municipal health administrative departments with districts shall, in accordance with the "Regional Health Planning", make full use of existing health resources, make unified planning, and reasonably set up community health service institutions. Strictly examine and approve in accordance with the provisions of the Measures for the Administration of Community Health Service Institutions, and supervise, manage, coordinate and guide the work of community health services in urban areas.
The district health administrative department is responsible for the preliminary examination and approval of the establishment of community health service institutions by medical institutions under its jurisdiction. And supervise, manage, inspect and guide the construction, work and implementation of relevant laws, regulations, rules and standards of community health service institutions under their jurisdiction.
Twentieth municipal and district health administrative departments shall, in accordance with the "Evaluation Standards for Urban Community Health Service Institutions in Hebei Province", regularly assess and evaluate the work of community health service institutions. Commend and reward units and individuals that have made outstanding achievements; Management confusion, ineffective implementation of the work, shall be ordered to make rectification within a time limit; After rectification, it still fails to meet the standard requirements, and its qualification to carry out community health services is cancelled.
Twenty-first established community health service institutions, which have not been approved and filed in accordance with these measures, shall re-declare before the end of 12. After passing the acceptance, they shall be issued with certificates and filed. 65438+Those that still do not meet the requirements at the end of February will be banned.
