(1) An application for maternity insurance benefits;
(2) the original and photocopy of the birth certificate issued by the family planning administrative department;
(three) the original and photocopy of the identity card and marriage certificate of both husband and wife;
(four) the original medical certificate issued by the medical institution, such as birth certificate, infant death certificate, medical certificate of birth or family planning operation, etc.
Unemployed workers shall submit the above materials (1) to (4), as well as the original and photocopy of unemployment insurance benefits Employment and Unemployment Registration Certificate issued by Xizang Autonomous Region Labor and Employment Service Agency. Unemployed spouses of employees must also submit the proof of no work unit issued by the employee's work unit or the village (neighborhood) Committee where the spouse is located.
The agency shall timely review the materials submitted by the applicant, accept those that meet the requirements, and within 30 days from the date of acceptance, verify the maternity insurance benefits that employees and their unemployed spouses should enjoy, and make a one-time plan; Do not meet the conditions, it shall promptly inform in writing.
