At the same time, the original materials should be submitted to the inspected unit or the designated filing unit for filing.

You need a physical examination before applying, and you can only apply for a health certificate after passing the physical examination. The specific process is as follows:

According to the list of personnel determined by the health supervision institution, go to the health inspection unit for preventive health inspection, and uniformly use the People's Republic of China (PRC) Preventive Health Checklist formulated by the Ministry of Health.

First, have a health check-up before applying for a health certificate.

1, 2 ID cards and one-inch color photos of individuals bareheaded, pay the medical examination fee at the toll booth, get the medical examination form, paste the photos (the other one is a health certificate), and fill in the basic information such as name and gender.
