You can't just apply for a health certificate with a copy of your ID card. To apply for a health certificate, you need to provide the original and photocopy of your ID card.
The method of obtaining health certificate is as follows:
1. Have a physical examination before applying for a health certificate;
2. Bring your ID card and two one-inch bareheaded color photos, and receive the medical examination form;
3. Pay the medical examination fee at the toll booth, paste photos and fill in basic information such as name and gender;
4. Complete all physical examination items;
5. If the physical examination results meet the conditions for issuing a health certificate, a health certificate shall be issued; If the physical examination results do not meet the conditions for issuing a health certificate, a health certificate shall not be issued.
Health certificate inspection items:
1. Routine examination includes: nutrition, development, blood pressure, height and weight;
2. Physical examination items: chest and lung auscultation, heart rate, heart rhythm, murmur, liver, spleen, etc.
3. Surgical examination items: lymph, thyroid, spine and limbs;
4. Test items: two and a half items of hepatitis B; Chest x-ray examination of heart, lung and diaphragm.
Industries that need to apply for health certificates:
1, engaged in food and drinking water production and management personnel, cosmetics production personnel, public places to directly serve customers;
2 food production and management personnel, drinking water supply and management personnel, cosmetics production enterprise production personnel;
3. Hotels, cafes, bars, cafes, public bathrooms, barbershops, beauty salons, swimming pools and direct customer service staff once a year.
4. The personnel who directly serve foreign customers have a physical examination every two years.
To sum up, if the physical examination results meet the conditions for issuing health certificates, the health certificates will be issued after passing the training on health laws and regulations organized by health supervision institutions. If the physical examination results do not meet the conditions for issuing a health certificate, a health certificate shall not be issued.

Food producers and business operators shall establish and implement the health management system for employees. Persons suffering from diseases that hinder food safety as stipulated by the administrative department of health of the State Council shall not engage in direct contact with imported food.
Food production and marketing personnel engaged in direct contact with imported food shall undergo annual health examination and obtain health certificates before taking up their posts.
