What is cervical trypanosomiasis? What are its symptoms?

Cervical spondylosis is a common clinical disease, which used to be more common in middle-aged and elderly people, and has a trend of rejuvenation in recent years; It used to be an occupational disease of accountants and teachers, but now it is a frequently-occurring disease in the office. The main symptoms of cervical spondylosis are neck pain and numbness of upper limbs, but modern medical research shows that cervical spondylosis can also produce symptoms such as headache, dizziness, tinnitus, eye swelling, chest tightness, shortness of breath and weakness of limbs. There are many methods to treat cervical spondylosis, such as massage, acupuncture, traction, physical therapy, local closure, medicine and so on. , as well as surgery, can have a good effect. However, if the patient does not cooperate with the treatment and does not pay attention to self-maintenance and functional exercise, it will delay the illness and easily relapse.

I. Maintenance

1. One third of one's life is spent in bed. The hardness of the pillow has a direct effect on the cervical spine. The best pillow should be able to support the physiological curve of the cervical spine and keep it straight. The pillow should be elastic, and the pillow core should be kapok, hollow high elastic cotton or chaff. Like to lie on your back, the height of the pillow is about 5 cm (the height after compression); I like to lie on my side, and my height is about 10 cm. In supine position, it is best to pad the lower edge of the pillow on the upper edge of the scapula to avoid making the neck empty. In fact, the real name of the pillow should be "pillow neck". Improper pillow often causes stiff neck, and repeated stiff neck is often a precursor to cervical spondylosis, so it should be diagnosed and treated in time. Also pay attention to the pillow mat, which is made of straw. One bamboo mat is too cold and the other is too hard, so it's better not to use it.

2. Keeping the neck warm The cold stimulation of the neck will make the muscles and blood vessels spasm and aggravate the neck pain. In autumn and winter, it is best to wear high-necked clothes; The weather is a little hot, so you should pay attention to prevent your neck and shoulders from catching cold when you sleep at night; In hot season, the air conditioning temperature should not be too low.

3. Correct posture The main reason for cervical spondylosis is that the posture of work and study is incorrect. Good posture can reduce fatigue and avoid injury. Bowing your head for too long will cause muscle fatigue, cervical disc aging and chronic strain, which will lead to a series of symptoms. The best posture for working at a desk is to keep your neck upright, lean forward slightly, and don't twist or tilt; If the working hours exceed 1 hour, you should take a break for a few minutes and do some neck exercises or massage; It is not advisable to lean your head against the bedside or sofa armrest to read or watch TV.

4. Avoiding neck injury can also induce this disease. Besides paying attention to posture, if you take a fast vehicle and encounter sudden braking, your head will be thrown forward, which will cause "whiplash-like" injury. So pay attention to protect yourself, don't doze off in the car, you can twist your body and lean forward when sitting in your seat; Cervical spine injury should be avoided in sports competitions; In the acute attack of cervical spondylosis, the activity of cervical vertebra should be reduced, especially to avoid turning around quickly, and neck brace should be used for protection if necessary.

Step 2: Exercise

Exercise of cervical vertebra should be cautious, and rapid rotation or swing without purpose should be avoided, especially in the acute stage of cervical spondylosis, vertebral artery type cervical spondylosis or cervical spondylotic myelopathy. The method we recommend is simple and easy, but to achieve the goal of preventing and treating diseases, we must persevere.

1. The head is neutral, bending forward to the limit and returning to the neutral position; Stretch back to the limit and return to the neutral position; Turn left to the limit and return to the neutral position; Turn right to the limit and return to the neutral position; The left side bends to the limit and returns to the neutral position; The right side bends to the limit and returns to the middle position. Slow down and push harder. During exercise, some patients can feel the murmur in the neck. If there is pain, they should reduce the number of exercises or stop exercising. If there is no pain, you can continue to exercise.

2. The head is in a neutral position, with hands and fingers crossed behind the neck, and the head moves back and forth slowly. At the same time, both hands vigorously resist the movement of the head and exercise the muscle strength at the back of the cervical spine.

Prevention of cervical spondylosis

To prevent cervical spondylosis, measures should be taken from both etiology and inducement to effectively reduce the incidence and prevent the recurrence of cured patients. The cervical vertebra is a part of the spine and should be prevented from the whole spine.

Prevention is one of the important contents of preventing cervical spondylosis. It is generally believed that the intervertebral disc of the spine begins to degenerate after it develops into an adult. In fact, the degree of degeneration of each intervertebral disc varies greatly from person to person. Observing from the age, Wei Zhi and others analyzed 100 asymptomatic cervical X-ray photos. The results showed that intervertebral disc degeneration and hyperosteogeny increased with age: 2/20 people in the group under 20 years old; 20~29 years old group, 2/20 people; 30~39 years old group, 4/20 people; 40~49 years old group, over 50 years old group, 16/20 people. According to Pan Zhiqing's research, infants and adolescents have the most chance of spinal trauma, so the prevention of cervical spondylosis should start from childhood. Strengthening physical exercise makes the soft tissue around the spine strong, which helps to enhance the stability of the spine. Pay more attention to preventing trauma and correcting bad posture in work and life. Besides trauma, the common inducing factors of cervical spondylosis include stiff neck, cold, excessive fatigue, forced posture work, bad posture and other diseases. The pathogenic factors of cervical spondylosis are complex, but they can generally be divided into internal causes (internal causes) and external causes (acute and chronic trauma), which can be mutually causal. Internal cause is the foundation of disease. What we are talking about here is external cause prevention.

(1) Strictly prevent acute head, neck and shoulder injuries: head and neck falls, bumps and whiplash injuries are all prone to cervical vertebra and its surrounding soft tissue injuries, which directly or indirectly cause cervical spondylosis, and should be actively prevented. Once it happens, it should be checked and thoroughly treated. Are some injuries easy to attract people's attention, such as falling asleep in the car, encountering sudden braking and suddenly leaning back, all of which will cause whiplash injury to the cervical spine; When someone is angry, twist the child's ear at will, and the child will twist his neck to defend himself, or pat the child on the back of the head. , can cause neck muscle and its surrounding soft tissue injury; Infants and young children have underdeveloped neck muscles and soft necks. If they hold their children too early or in an inappropriate posture, it is easy to cause hyperextension cervical spine injury. Some teenagers' sports activities fail to get to the point or do not pay attention to the warm-up activities before sports, such as pushing cattle, handstand, forward rolling, neck riding and so on, which may cause sports injuries. Prevention of trauma is a powerful measure to prevent spinal degeneration. Once trauma happens, besides soft tissue injury, it is necessary to treat the dislocation of cervical facet joint in time to prevent the development of cervical spondylosis.

(2) Correcting bad posture in daily life and preventing chronic injury: Chronic strain of neck and shoulder soft tissue is the pathological basis of cervical spondylosis, and bad posture in daily life is one of the main causes of chronic strain, so correcting bad posture in daily life is of great significance for preventing cervical spondylosis. For example, some people like to lie prone and can only twist their heads to one side in order to breathe, which will cause 1~4 cervical sprain. Lateral curvature of the neck axis will lead to dizziness, headache, eyes, ears, nose and throat when decompensated. Because it destroys the normal mechanical imbalance of cervical vertebrae, it will accelerate the intervertebral disc degeneration of each cervical vertebra; Some people usually have a good posture, but when reading novels and watching TV, they are used to leaning their heads against the railing of the bed or the armrest of the sofa, causing neck flexion, back flexion and waist torsion, which will lead to instability of this section of the spine due to the injury of the intervertebral ligament of the spine; When a woman takes her child to sleep, she often lies on her side facing the child. If the pillow is out of height, the cervical and thoracic vertebrae will be in a stressed position, forming lateral curvature, which may lead to spinal diseases.

(3) Reasonable use of pillows: pillows are the protective tools for neck columns. An adult sleeps six to nine hours a day, which means every day? ~ 1~3 is spent in sleep (on the pillow), so the pillow must be suitable for the physiological requirements of the neck. After deep sleep, the neck and shoulder muscles are completely relaxed, and the normal relationship of intervertebral structure is maintained only by the elasticity of intervertebral ligament and joint capsule. If the pillow with inappropriate height is used for a long time, the cervical spine will be excessively flexed, and the ligaments and joint capsules here will be damaged by stretching, which will lead to cervical instability and joint dislocation, and then develop into cervical spondylosis. Such patients often show neck discomfort, stiff neck, dizziness, headache or intractable insomnia during sleep or after waking up in the morning. Reasonable pillow is very important for the treatment and prevention of cervical spondylosis, which cannot be replaced by drug treatment, but it should be applied for a long time. A reasonable pillow must have two items: scientific height and comfortable hardness. Scholars at home and abroad attach great importance to the height of pillows and put forward a variety of data. Our research suggests that pillows should not be too high or too low. Most people use their own jaw-shoulder line (the distance from the mandibular angle to the acromion) or the transverse diameter of the palm as the height of lateral or supine position, which is suitable for most people; A few people need proper high pillows, such as spinous process dysplasia and kissing spines. Too low a pillow will aggravate the symptoms. Pillows should have appropriate elasticity or plasticity, not too hard, kapok or chaff is better, and can form a saddle shape after application.

Good sleeping posture is very important for the health care of the spine. The trunk, shoulders and pelvis of the human body have large transverse diameters. When lying on your side, your spine bends due to the influence of the mattress. If you lie on your side for a long time, your spine will gradually bend sideways. If you wake up lightly, your back will be stiff and uncomfortable, and you need to get up and exercise to return to normal. In severe cases, you may suffer from spinal diseases. Sleep should be mainly supine, supplemented by lateral lying, alternating left and right, and the left and right knee joints are slightly flexed when lying on the side. Prone, half-lying, half-lying, or twisting up and down to sleep are all bad sleeping positions, which should be corrected in time. Put your head in the center of the pillow to prevent stiff neck. Patients with spinal diseases should use wooden beds, and spring beds are not conducive to the physiological balance of the spine.

(4) Prevention of chronic strain: Due to work needs, some types of work need special posture or work in forced posture for a long time. If we don't pay attention to it, it is prone to chronic strain and gradually develop into spinal diseases. For example, people who look at microscopes for a long time, tunnel workers, accountants, sewing and embroiderers, dentists, airplane mechanics, typists, telegraphers and other workers who bend their necks, twist their necks and shrug their shoulders, and people who study at their desks for a long time, if they do not pay attention to the height of desks and chairs to adapt to their bodies, and they do not pay attention to the balanced movement after work (posture opposite to the working posture, such as stretching at their desks) to prevent chronic strain, except for

Sleeping in the car and braking suddenly are easy to cause cervical spine injury, so try to prevent it. Swimming in shallow water often causes serious cervical injuries, so diving should be in deep water.

(5) Prevention of the elderly: Most elderly people over 50 years old have degenerative changes in their spine, so more attention should be paid to the prevention of spinal diseases. Pillows must meet personal specifications (healthy pillows are divided into five specifications: extra large, large, medium, small and extra small, and men use the same number of pillows according to their personal shirt numbers; Ladies use smaller pillows according to the size of their shirts), and they should pay attention to using pillows at home or out; When the weather is cold, pay attention to the warmth of the neck and waist, and reduce bad postures such as necking, shrugging and bending. In winter, we should pay attention to prevent the neck and shoulders from catching cold, especially when sleeping, to avoid stiff neck, cervical spondylosis and scapulohumeral periarthritis caused by cold stimulation. When talking to people, watching TV, watching movies or reading newspapers, you should try to look at them positively and don't bend your neck too much. In short, it is necessary to maintain the normal physiological curvature and integrity of the spine and prevent cervical spondylosis caused by poor posture.

In recent years, studies at home and abroad have proved that cardiovascular and cerebrovascular diseases and chronic diseases that endanger the lives of middle-aged and elderly people are related to the spine. Therefore, as an elderly person, it can also prevent hypertension, coronary heart disease, arrhythmia and cerebrovascular diseases. The most active way to treat diseases is to nip in the bud, especially in the era of increasing therapeutic drugs, and it is particularly important to attach importance to preventive medical education.

Daily health care of lumbar spine, prevention and non-surgical treatment of lumbar diseases

What is the relationship between sleeping and low back pain?

Author: Dr. Zhang Li, Deputy Chief Physician of Orthopaedics, Peking University Third Hospital.

Bed rest can relieve lumbar muscle spasm and make lumbar muscles and intervertebral discs fully rest and relax. You should choose a hard bed or a hard elastic bedding such as Simmons on a wooden bed to sleep, which is comfortable and can make your waist fully rest and relax. Traditional brown stretching or nylon stretching bed, steel stretching bed and steel stretching camp bed, etc. When people lie on it, their bodies will be in a state of low, middle and high angles because of their weight. It's a brown elastic bed that has been used for a long time, and its elasticity is even worse. When people lie on it, the waist muscles are still in a state of spasm, which makes the intervertebral disc not fully rested and relaxed, which is very harmful to the waist. After sleeping in this bed, many people will feel back pain and weakness after getting up the next morning. In the long run, it is easy to cause lumbar muscle strain and induce lumbar disc herniation. In addition, soft sofas or Simmons mattresses are too soft or have poor elasticity, which will have the same shortcomings as traditional stretchers. Many people can induce lumbar muscle strain by sleeping in these unsuitable beds for a few days, and their back is sore and weak all day, and they can't find the reason for it for a long time. After changing into a hard bed, lumbar muscle strain can miraculously recover.

It is better to lie on your side when sleeping, and the lower limbs are slightly flexed, which can reduce the pressure in the lumbar intervertebral disc and make the lumbar muscles fully rest.

It is very important to exercise your back muscles.

Author: Dr. Zhang Li, Deputy Chief Physician of Orthopaedics, Peking University Third Hospital.

Lumbar dorsal muscle is one of the important structures to maintain the stability of lumbar spine. Strengthening the exercise of nape muscles is helpful to maintain and enhance the stability of lumbar spine, thus delaying the degeneration of lumbar strain and effectively preventing the occurrence of acute and chronic lumbar injury and low back pain. This is especially important for patients who have suffered from acute and chronic lumbar muscle injury, lumbar muscle fasciitis, lumbar muscle strain or lumbar disc herniation, but are currently in remission, and it is particularly important to prevent the recurrence of the disease. People who stay in bed or wear waist circumference therapy because of low back and leg pain have no exercise and pressure, which may lead to disuse atrophy and waist weakness in the long run. Therefore, it is necessary to strengthen the exercise of lumbar muscles.

The method of back muscle exercise is simple and low cost, and everyone can do it by himself every day. Other treatments have basically no adverse reactions.

When exercising, you can lie prone on the bed, remove the pillow, hold your chest out behind your hands, make your head and chest leave the bed surface, straighten your knees at the same time, and force your thighs to leave the bed surface for 3 ~ 5 seconds, then relax your muscles for 3 ~ 5 seconds. This method is commonly called "Little Swallow" or "Little Swallow". For people with weak waist muscle strength or obesity, the above method is laborious. You can use the "five-point support" method to exercise, lie on your back on the bed, remove the pillow and bend your knees, elbow and back against the bed, lift your abdomen and buttocks upward, support your whole body weight by five points of shoulders, elbows and feet for 3-5 seconds, then relax your waist muscles, and rest your hips for 3-5 seconds in a cycle. You can choose a suitable method to exercise according to your actual situation.

The frequency and intensity of back muscle exercise vary from person to person, and it can be practiced a dozen times to a hundred times a day, and can be completed in 3 ~ 5 groups. It should be gradual, and the daily exercise can be gradually increased. If you feel waist pain, discomfort, stiffness, etc. On the second day after exercise, the intensity and frequency of exercise should be appropriately reduced, or exercise should be stopped to avoid aggravating symptoms; Don't suddenly exert yourself too hard during exercise, in case you sprain your waist by exercising your lumbar muscles.

If you have symptoms such as low back pain, stiffness and discomfort, you should stop or reduce back muscle exercise; When you have an acute attack of low back and leg pain, you should rest in time and stop practicing, otherwise it may aggravate the original symptoms.

The waist health care movement should pay attention to science.

Author: Dr. Zhang Li, Deputy Chief Physician of Orthopaedics, Peking University Third Hospital.

After the symptoms of low back and leg pain appear, if there is enough rest or treatment, the strained waist muscles and ligaments can be recovered and the symptoms can be alleviated to a certain extent; On the other hand, if the strain continues, the aseptic inflammatory reaction of local tissues will continue to increase, the elasticity of muscles will become worse and worse, and the vertebral bodies and intervertebral discs will also be stimulated by strain, forming a vicious circle. Therefore, after suffering from low back and leg pain, the scientific method should be the combination of static and dynamic, treatment and health care. In the case of acute attack, you should go to the hospital in time, have a full rest and even rest on the hard bed. After the symptoms are stable or relieved, it is necessary to strengthen back exercise and proper back activities.

Proper exercise and back movement can enhance the strength and elasticity of waist muscles, ligaments, joint capsules and other tissues, improve the relationship between neck and surrounding soft tissues, improve the function of lumbar intervertebral joints and enhance the stability of lumbar spine. At the same time, proper soothing activities are also helpful to improve local blood circulation, relieve fatigue, relax muscles and tendons, promote blood circulation and remove blood stasis, and help to relieve local pain and tissue adhesion; At the same time, it can also relieve muscle tension and spasm and prevent disuse atrophy of waist muscles; It can also prevent and slow down lumbar strain and effectively delay the process of lumbar degeneration; It can relieve the bad stimulation of nerve roots caused by various static compression factors such as bone spurs and lumbar instability factors, and prevent and alleviate symptoms such as low back and leg pain caused by lumbar strain and degeneration.

However, it should be noted that the exercise of the waist and back should be moderate, the speed should not be too fast, the duration and intensity should not be too great, and it should not be too frequent. Should be based on the comfort of the waist after exercise, do not aggravate the original symptoms, do not appear waist pain and discomfort; If the symptoms of lumbago and leg pain are acute, those with severe lumbago and leg pain should rest mainly instead of practicing.

It is also incorrect to shake the waist repeatedly and excessively. Inappropriate exercise of the waist can not only fail to achieve the purpose of health care, but also accelerate the strain and degeneration of the lumbar spine due to excessive repetitive exercise of the lumbar spine, which may induce low back pain in healthy people without diseases or aggravate the symptoms of patients with existing symptoms.

As Mr. Qian said earlier, when the low back pain is still very heavy, shaking your waist inappropriately and even wanting to jog will of course backfire.

The correct method should be to take appropriate bed rest, local lumbar braking, oral anti-inflammatory and analgesic drugs, lumbar traction, physical therapy and other treatment methods during the acute episode of low back pain. After the symptoms are obviously relieved or disappeared, the back muscles will be exercised step by step to alleviate moderate lumbar activities.

For bedridden patients, we should strengthen the activities of bedridden limbs without aggravating the symptoms of low back and leg pain, so as to prevent or slow down the disuse atrophy of limbs and reduce the joint stiffness and ligament adhesion of limbs.

For the exercise of the waist after going to the ground, it is advisable to stand with your feet apart and shoulder width. First, slowly move the waist back and forth, bend left and right, and rotate left and right to the maximum. Activities in all directions can be repeated for 5 ~ 15 cycles, not too many. You can also hold your waist with your hands between sitting and working, and move gently in all directions, which can also appropriately relieve the nervous ideological pressure when sitting and working.

Lumbago and leg pain can be cured, and the purpose of its treatment is to eliminate symptoms rather than remove bone spurs.

Author: Dr. Zhang Li, Deputy Chief Physician of Orthopaedics, Peking University Third Hospital.

Lumbar degeneration and hyperplasia is a gradual physiological or pathological degeneration and aging process. In this process, besides degeneration of intervertebral disc, stenosis of intervertebral space, hyperosteogeny of anterior and posterior edges of vertebral body and articular process, a series of changes such as congestion, swelling, fibrosis, calcification or ossification of articular capsule and its surrounding ligaments also occur, which can stimulate and compress lumbar nerve roots, causing low back pain or radiation pain, numbness of lower limbs and other symptoms. At present, there is no effective method to reverse the degeneration and hyperplasia of lumbar spine or inhibit its further development. In other words, the degeneration and hyperplasia of lumbar spine is related to human natural aging, which is irreversible and does not need treatment.

Various non-surgical treatment methods used in clinic are mainly aimed at the reactive changes of surrounding tissues after lumbar degeneration and hyperplasia, and through proper rest, lumbar immobilization, anti-inflammatory and analgesic drugs and physical therapy, the reactive inflammation, ischemia and swelling of surrounding tissues are alleviated, and the stimulation and oppression of lumbar nerve roots are slowed down, thus improving related clinical symptoms. If the symptoms disappear completely, we say that the neck, waist and leg pain has been cured clinically.

In this sense, proliferative and degenerative diseases such as lumbar spine spurs can neither be treated nor treated; Lumbago and leg pain can be treated and cured. The treatment of lumbago and leg pain is only aimed at the clinical symptoms caused by lumbar degeneration, rather than treating hyperosteogeny and eliminating bone spurs. The so-called clinical cure is to eliminate all kinds of clinical symptoms of low back and leg pain caused by lumbar spur, hyperosteogeny or lumbar muscle strain, rather than eliminating spur. Therefore, the treatment standard of low back and leg pain should be evaluated from clinic, not from image X-ray.

Of course, for lumbar disc herniation or lumbar spinal stenosis, if non-surgical treatment can not effectively alleviate the symptoms of patients, you can choose surgical treatment. During the operation, the bone spurs or protruding intervertebral discs that compress or stimulate the lumbar nerve roots can be directly removed, and the unstable lumbar spine can be restored to new stability through bone grafting and fusion, thus reducing the stimulation to the lumbar nerve roots and relieving the symptoms of low back and leg pain. But the operation is only aimed at the causes of symptoms, not to remove all the spurs of the lumbar spine.

When there are symptoms of low back and leg pain or the original symptoms are aggravated, you should go to the hospital for treatment. According to the doctor's advice, you can choose the appropriate treatment. The treatment of low back and leg pain can be divided into two categories: non-surgical conservative therapy and surgical therapy. When making the treatment plan, we should not only make a comprehensive analysis according to the patients' different clinical types, course length, severity of illness, health status, etc., but also make timely adjustments according to the patients' different reactions to the treatment effect during the treatment. After correct and timely treatment, the symptoms of low back and leg pain can disappear or be alleviated, so low back and leg pain can be cured.

Don't be nervous if you have low back pain. Under the current medical level, about 95% of patients can be cured by formal treatment, but you should seek medical treatment as soon as possible, don't mistake folk remedies and quacks, and don't let people who have no experience in treating low back and leg pain massage casually. Due to improper massage operation, it is not uncommon for patients to get worse or even become paralyzed or incontinent.

At present, it is considered that lumbago and leg pain is a self-limiting disease. Most clinical diseases such as lumbar disc herniation, lumbar muscle strain, lumbar supraspinous ligament inflammation and so on. Non-surgical conservative treatment can obviously alleviate and cure, so non-surgical treatment should be the first choice for treatment; If the conservative treatment of lumbar disc herniation is ineffective, or the lumbar spinal canal is narrow, the patient's lower limbs are numb and painful, and surgical treatment is more appropriate. At present, the effect of surgical treatment is basically satisfactory, and most patients can recover their good life and work ability, which is expected to be cured. Therefore, patients who have been diagnosed with low back and leg pain need not be afraid and anxious. They should cooperate with the doctor and treat according to the doctor's advice. Most patients with low back and leg pain can be cured by choosing the right treatment and rehabilitation methods.

How to choose treatment for patients with lumbar disc herniation?

Author: Dr. Zhang Li, Deputy Chief Physician of Orthopaedics, Peking University Third Hospital.

Non-surgical treatment is the first choice for most patients with chronic low back pain caused by lumbar disc herniation, and the effect is good. Non-surgical treatment is simple and convenient, with few complications and low cost, which is easy for patients to accept and most patients with lumbar disc herniation are satisfied. Most patients are expected to be cured by correctly applying various non-surgical treatments; Even for patients who need surgical treatment, it is essential to carry out various effective non-surgical treatments before and after operation.

A few authors are uncertain about the effect of long-term non-surgical treatment, or they have certain effects but recurrent symptoms, so surgical treatment should be considered; A small number of patients with lower limb muscle paralysis and dysfunction of urination and defecation due to nerve root compression should be operated as soon as possible; In addition, a few patients have acute symptoms and severe pain, which seriously affects their lives, especially those who have difficulty falling asleep at night. It is estimated that non-surgical treatment is difficult to be effective in the short term, and surgery should be considered as soon as possible.

For patients with simple lumbar disc herniation who need surgery, most of them just take out the protruding disc from the back of the lumbar spine to reduce the compression on the nerve roots, which can quickly and effectively relieve the symptoms of low back and leg pain, and the curative effect is satisfactory. The operation technique is mature, the operation is relatively simple, the complications are few, the incision is small, and the damage to the stability of lumbar spine is small. Most patients are satisfied with the short-term and long-term curative effects, and can quickly return to normal work and life after operation with low recurrence rate. Especially in large hospitals with strong technical force, the effect of surgical treatment should be more guaranteed.

For patients with lumbar disc herniation complicated with lumbar instability, in addition to the above-mentioned resection of the herniated disc, pedicle screw fixation and bone graft fusion are often needed, similar to the operation of lumbar spinal stenosis.

What role does waist circumference play in treating lumbocrural pain?

Author: Dr. Zhang Li, Deputy Chief Physician of Orthopaedics, Peking University Third Hospital.

The first purpose of lumbar braking is to rest the lumbar muscles. Second, after the lumbar vertebrae are properly fixed and braked, it can limit the excessive movement of the lumbar vertebrae, reduce the adverse stimulation of the lumbar nerve roots caused by compression objects such as lumbar hyperosteogeny and disc herniation, alleviate the traumatic reaction of intervertebral joints, relieve and improve the pressure state of intervertebral space, and reduce the persistent injury and strain, which is conducive to the regression of tissue edema and injury repair, and can also consolidate the curative effect and prevent recurrence.

Lumbar braking mainly uses various braces, such as waist circumference, which is suitable for most patients with low back and leg pain. It is best for most patients with low back and leg pain to insist on using lumbar brace fixation during treatment, especially after lumbar surgery. You can wear a lumbar support when you get up and take it off when you rest in bed.

For patients with low back and leg pain, besides bed rest, lumbar immobilization is the second basic conservative treatment measure during non-surgical conservative treatment, which is very important. Lumbar support is more meaningful for some patients who can't stay in bed for a long time. Some patients can also improve their symptoms by simply protecting their waist, but we still advocate that waist protection should be combined with bed rest, physical therapy, traction, anti-inflammatory and analgesic drugs, promoting blood circulation and removing blood stasis, detumescence and analgesic drugs and other local topical drugs, and the effect is better.

At present, many medical device manufacturers have made various waistlines or lumbar supports for patients with different body types and requirements to choose from. Patients can choose according to the thickness of the waist and their different requirements. Most of them are light in weight, strong in strength and easy to wear and disassemble, and some manufacturers can tailor them according to the requirements of different patients.

There are advantages and disadvantages to using waistline or lumbar support. The advantage is that the effect of relieving symptoms is reliable, and the disadvantage is that the exercise opportunity of waist muscles is weakened. Long-term application will cause waist muscle atrophy and waist stiffness. Therefore, the wearing time should not be too long. When the symptoms are serious, it can be applied for a short time, and the drug should be stopped in time after the symptoms are gradually relieved. If there is no discomfort at other times, there is no need to wear it often. It is best to give the waist muscles proper exercise opportunities. In the application of waist circumference, it is necessary to strengthen the exercise of lumbar muscles and restore their strength, which is conducive to the recovery and strengthening of lumbar stability.
