Application for health certificate:
Take two one-inch color photos with ID card and personal bareheaded, get the physical examination form, pay the medical examination fee at the toll booth, paste the photos (the other one is for health certificate), and fill in the name and gender.
Physical examination place of health certificate: local disease prevention and control center or medical and health institution approved by health administrative department to undertake preventive health examination. If this work is carried out without approval, the health supervision institution will not accept the medical certificate issued by it.
