With the continuous development of Internet technology, with the normalization of epidemic prevention and control and the continuous support of national policies, Internet medical care has ushered in a new period of vigorous development. As a new medical model, while developing rapidly, it also brings new challenges to the service quality and management of Internet medical platform.

On the one hand, the Internet platform extends medical services from the hospital to the outside of the hospital, from offline "one-to-one" management to continuous "one-to-many" management, which not only makes full use of medical resources, but also improves the efficiency, breadth, depth and overall level of medical services.

On the other hand, the management of chronic diseases has risen to the national strategic level. It is the goal and responsibility of the whole society to reduce the premature mortality rate of chronic diseases and control the burden of chronic diseases. As a new format, Internet hospitals are still in the initial stage, and need mutual promotion from Industry-University-Research to accumulate strength for the development of the industry.

First of all, Internet medical care began to enter the deep water area, and more and more doctors not only received consultation on the Internet platform, but also went deep into the whole process management of diseases and were responsible for the final results of patients.

In this year's first Internet Chronic Disease Management Competition sponsored by the Chinese Medical Doctor Association and co-organized by the Medical Association, we collected thousands of Internet chronic disease management cases from four disciplines: tumor, infection, respiration and endocrinology. One example is a doctor from the Department of Infectious Diseases of the Fifth People's Hospital of Ganzhou City, Jiangxi Province, who followed up an HIV-infected person infected with hepatitis B on the Internet. When the patient's liver function index is obviously abnormal, through the support of follow-up, medication, diet education and mental health consultation on the platform of medical association, the patient can successfully prevent HIV-related opportunistic infections and effectively avoid the occurrence of cirrhosis and liver cancer.

The case was finally rated as the first infection group in this competition. In the case summary, the doctor particularly emphasized two advantages of the Internet platform in patient management.

The first is to solve the problem of drug accessibility. "Especially in the case that there is no (antiviral drug) dotiravir in major hospitals and pharmacies in Ganzhou, purchasing drugs through the medical alliance platform can not only meet the needs of patients, but also strengthen the supervision of patients' treatment compliance. "The second is to achieve efficient interconnection between doctors and patients." Because patients with HIV /HBV co-infection need long-term management, and there are many patients in offline hospitals, which can not be managed exclusively and effectively, Internet hospitals can make up for this deficiency and communicate with doctors and patients one-on-one, which greatly improves patients' medical experience, improves patients' treatment compliance and effectively guarantees patients' health. "

Another prominent phenomenon in the development of the industry is that emerging technologies have begun to deeply empower medical and health services, and smart devices and digital therapies have emerged one after another. Some data that used to require patients to return to the hospital for follow-up can now be collected in real time through wearable devices, and can be fed back to doctors in time no matter where the patients are. Some diseases that used to be relieved by drugs can now effectively interfere with patients' daily behavior through the App on mobile phones, reducing or even eliminating patients' dependence on drugs.

In this process, the connection between patients and data plays a vital role. Medical data is necessary for the development of the industry, but it is currently lacking. Internet technology provides a new channel for data collection. The data ultimately comes from patients, and a connection platform like Medical Union can receive timely feedback from patients on new drugs, new tests, new behavioral interventions and other treatment methods. In other words, the Internet platform opens another door outside the laboratory for doctors and researchers to conduct medical experiments.

At the inauguration ceremony of the Academic Committee of the Medical Union in June this year, I was very encouraged by what Professor Cheng Shujun, a leading expert in our academic Committee, a famous expert in tumor etiology and an academician of China Academy of Engineering, said-people know a lot about life and diseases, and one of the most important reasons is that we have limited data and observed cases. With the advent of the information age, in the process of the development of life science and human evolution, people will certainly see many phenomena and data that they have never seen before. The revelation of these phenomena and data will inevitably promote the development of medical science, even a major revolution.

From 20 19, the government work report first put forward the development of "Internet+medical health", and now, "Internet+medical health" has appeared in the government work report for three consecutive years. At the same time, we note that this year's government work report puts forward "promoting normative development" for the first time. At the annual meeting of Boao Forum for Asia in April, the relevant leaders in National Health Commission also reiterated that at present, from the policy level, the first thing is to encourage and support the "Internet+medical care" service, and at the same time, to standardize and do a good job in two dimensions.

This means that as Internet medical care gradually moves towards deep water, standardization will become the "golden key" to unlock the further development of the industry. Frankly speaking, Internet medical care has developed for so many years, and there has been no breakthrough in many places. One of the key bottlenecks is that when the industry is driven by the demand for epidemic prevention and control, some new infrastructures have been completed and some new scenes have emerged, and many practitioners do not know how to use these medical resources efficiently and reasonably. From last year to now, we have seen the number of Internet hospitals in China soar from several hundred to more than 1600. However, in a recent program, CCTV pointed out the dilemma of the current development of Internet hospitals-building instead of using them.

As a graduate of West China who studied medicine for eight years, this reminds me of the development of West China: for more than 40 years, an entrepreneurial hospital has gradually emerged from remote areas, in which many creative solutions have been used. One of the most famous methods is that a hospital should start with discipline construction, that is, before building a hospital and developing a discipline, we should ask ourselves what we can do in this discipline.

Therefore, when we have a large number of Internet and medical resources, in retrospect, Internet hospitals must have a real, standardized and authoritative disease management method, which we call clinical pathway. I once compared this job to "sitting on the bench", and we started thinking about "sitting on the bench" a few years ago. What excites me is that some of our work has blossomed this year.

In May this year, Professor Xiao Xinhua from the Department of Endocrinology of Peking Union Medical College Hospital, Professor Guo Lixin from the Department of Endocrinology of Peking University Hospital, and other more than ten domestic experts, together with the Internet Hospital of Medical Union, worked out the China Expert Knowledge of Online Diabetes Management in Internet Hospitals, which was published in the Frontier of China Medicine. This is the first published online management expert knowledge of diabetes in Internet hospitals in China, including the use of intelligent devices such as smart blood glucose meters, smart sphygmomanometers and smart bracelets to conduct all-round data monitoring on patients, breaking the original offline time and geographical barriers; With the help of intelligent information technology, medical staff can manage patients' life cycle more conveniently by using fragmentation time.

In July, the first Expert's Guide to Internet Management of Adverse Reactions Related to Common Drug Therapy for Patients with Liver Cancer, supported by the Medical Association, was published in the core journal Electronic Journal of Liver Cancer. The guideline screened out 17 kinds of drug-related adverse reactions of liver cancer patients suitable for internet management, which filled the gaps in the existing guidelines and knowledge of * * * *, and was highly praised by experts such as Professor Cai, deputy director of the National Cancer Center and vice president of the Cancer Hospital of China Academy of Medical Sciences. At the 4th China Cancer Summit Forum just held at the end of May 438+10, the cartoon version of "Guidelines for Common Adverse Reactions of Cancer Drugs" compiled by the Medical Association as an academic supporter, together with Professor Cai and several experts in the field of cancer was officially released.

In the future, the research results of the Medical Association in the fields of AIDS and chronic kidney disease will be published one after another. We firmly believe that if we really want to develop Internet medical care, we must do a solid job in discipline construction. It is not easy. The discipline construction of a new carrier or a new resource is very complicated, which requires top-notch brains and top-notch medical cognition, but we can't worry, let alone worry.

Today, the medical association has gone through seven years, and it has crossed many challenges and carried many expectations. Academician Cheng Shujun encouraged us that from the perspective of using information to promote the medical revolution, the medical association should have great room for development in the future. I hope that with the development of Internet and informatics, we can promote people's understanding of diseases and promote the healthy development of mankind.

This coincides with our new mission of "extending the healthy life span of all mankind for one year" when the organizational structure was upgraded in April this year. To shoulder such a mission, we must adhere to the strategic direction of serious illness medical treatment. The essence of medical treatment is to prolong the healthy life of human beings, and the quality of disease control and management determines the quality of life and survival dignity of patients. The medical association does not want to be just a simple platform for online consultation, online consultation and information dissemination. We want to participate in the real medical environment of prevention, diagnosis, treatment and rehabilitation, which is also a core starting point for me to choose to start a business at that time.

We know that moving towards the deep water area of disease whole process management means deeper service mode, higher technical threshold and higher integration difficulty, and ultimately we are responsible for the patient's results. What we urgently need to discuss is how to integrate Internet medical resources with traditional medicine and authoritative cognition after many breakthroughs, and how to assemble high-quality medical supply, testing supply, screening supply, nutrition supply and other resources and put them in the corresponding position in the management process. This is not destined to be a fast and smooth road, but requires us to stick to our initial intentions and work hand in hand with all partners and the whole industry.

Fortunately, our strategy and persistence have won the recognition and support of top partners in medicine, commerce and other fields. The academic committee of our hospital is led by Academician Cheng Shujun and Liu Jiaoshou, a famous cardiovascular expert and former president of the World Hypertension Union. The first group of experts also includes dozens of academic leaders and experts and professors who have been engaged in various diseases for many years, covering many disciplines such as cancer, endocrine, infection, cardiovascular and ophthalmology. Since 20 14, we have received the support of capital partners such as Sequoia, Tencent, CLP Data, CIC Cai Zhong, China Biopharmaceutical, etc. Mei Sen Medical, which we invested last year, is committed to establishing the China model of digital treatment. In less than two years, it has been officially recognized by more than 100 academic hospitals.

If you don't accumulate steps, you will be thousands of miles away; If you don't accumulate small streams, you won't be a river. Under the guidance of the strategy of "Healthy China", we look forward to working with more industry partners to promote the transformation of China medical industry from "treatment" to "health prevention and disease management" step by step. In the future, we also hope to unite various academic forces, share the past experience of the medical association in the construction of online patient management system, standardized management process outside the hospital, discipline construction, etc., and discuss, promote and promote the standardized development of the Internet medical industry with all sectors of society, so as to better empower doctors and serve patients with the advantages of convenience, accessibility and intelligence of Internet hospitals.
