National health record

In 2009, the Ministry of Health launched the National Health Record Plan. The plan requires that by the end of 2009, the pilot filing rate of rural residents' health records nationwide will reach 5%, and the pilot filing rate of urban residents' health records will reach 30%. On the 3rd, the Ministry of Health issued the "Guiding Opinions of the Ministry of Health on Standardizing the Management of Urban and Rural Residents' Health Records" (hereinafter referred to as the "Opinions"), proposing a specific timetable for establishing residents' health records. The Opinions pointed out that the establishment of health records for urban and rural residents should be under the unified leadership of the county (city, district) health administrative departments, and be specifically responsible for urban and rural grassroots medical and health institutions such as community health service centers, community health service stations, township hospitals and village clinics. It is necessary to focus on key points and gradually establish health records for the elderly, patients with chronic diseases, pregnant women and children aged 0-3, and gradually expand them to the whole population.

The Opinions pointed out that health records should be stored in urban and rural primary medical and health institutions. When providing medical and health services, primary medical and health institutions in urban and rural areas should search and consult residents' health records, record, supplement and improve health records in time, understand and master the dynamic changes of residents' health within their jurisdiction, adopt appropriate technologies and measures to carry out health education, prevention, health care, medical care and rehabilitation services in view of the health problems found.
