How to collect health history?
The collected data should include the patient's physiological, psychological, social and cultural status, generally including: general information, current health status, past health status, family history, physical examination results, recent laboratory and other examination results, psychological status, social and cultural status and so on. The methods of collecting data are as follows: (1) Conversation is purposeful communication. The purpose is to help obtain patient health information; Communicate feelings and establish good interpersonal relationships; Timely feedback to patients about their illness, examination, treatment, rehabilitation and other aspects of information; Provide psychological support for patients. (2) Observation is a method to obtain health information by using the senses. Through observation, the patient's physiological, psychological, spiritual, social and cultural information can be obtained. (3) Physical assessment is a method of systematically using physical examination means and techniques such as sight, touch, knock, hearing and smell to examine patients and collect data. The condition of the affected system should be checked according to the characteristics of the patient's disease. (4) Access includes access to the patient's medical records, nursing records and various auxiliary examination results.
