1, two years after the revocation of the medical institution license, you can reapply. If you meet the conditions, a medical institution license will be issued. Failure to meet the conditions will not be registered. The registration authority shall, within 20 working days from the date of receipt of the application for registration, notify the employing organization and the applicant in writing, and explain the reasons. If the applicant has any objection, he may apply for administrative reconsideration or file an administrative lawsuit with the People's Court in accordance with the law.

Medical institutions to practice, must be registered and receive "medical institutions practice license.
Article 16
Applicants for registration to practice in a medical institution shall have the following conditions:
(1) an approval letter for the establishment of a medical institution;
(2) conformity with the basic standards of a medical institution;
(3) a suitable name, organization and premises;
(4) appropriate to the business they carry out funds, facilities, equipment and professional health technicians;
(v) have appropriate rules and regulations;
(vi) be able to independently assume civil liability.
Article 17
The registration of the practice of medical institutions shall be handled by the administrative department of health of the people's government which approves its establishment. The registration of practice of medical institutions set up in accordance with the provisions of Article 13 of these Regulations shall be handled by the administrative department of health of the people's government of the province, autonomous region or municipality directly under the central government where it is located. Institutions, enterprises and institutions set up for internal staff services clinics, clinics, health centers (rooms) of the practice of registration by the local county-level people's government health administrative department.
Two, apply for registration of physician practice, what materials should be submitted
1, physician practice registration application review form;
2, the last 6 months, 2-inch white background, bareheaded front half-body photo;
3, medical, preventive, and health care institutions, proof of employment;
4, the provincial level and above, other materials prescribed by the health administrative department.
