Application for Class B large medical equipment configuration license shall have the following conditions: (a) in line with the Class B large medical equipment configuration planning; (b) with a license to practice and set up the appropriate diagnostic and treatment subjects; or with the relevant provisions of the requirements of the legal person engaged in the provision of medical services, other qualifications; (c) with the functional positioning, clinical service needs, with an application for (C) with the functional positioning, clinical service needs, with the application of large-scale medical equipment compatible with the technical conditions, supporting facilities and have the appropriate qualifications, capabilities of professional and technical personnel; (D) medical quality and safety system is sound. Article 8 The applicant shall submit paper and electronic application materials in seven copies to Shaanxi Provincial Government Affairs Hall (hereinafter referred to as the Government Affairs Hall), and the paper application materials and electronic application materials shall be consistent. The electronic application materials are submitted through the Shaanxi government affairs service network.
Article 9 The paper application materials submitted by the applicant include: (a) Class B large-scale medical equipment configuration license application form (Annex 2); (b) a copy of the applicant's license to practice, or meet the requirements of the relevant provisions of the other legal person engaged in health care services copy of the qualification; (c) a copy of the unified social credit code certificate (or organization code certificate); (d) with the application for the allocation of large-scale medical equipment configuration (d) with the application for the configuration of large-scale medical equipment corresponding to the technical conditions, supporting facilities and professional and technical personnel qualifications, ability to copy the material.
