Fishbone triggered perianal abscess

On November 15 last year, living in Nanjing, Wang Jun (a pseudonym) and a few good friends to eat together, talking and laughing, not paying attention, eating fish accidentally put a fishbone to eat in, because there is no card to the throat, Wang Jun is also very grateful.

Wang Jun rushed to a clinic not far from his home, after anti-inflammatory cleaning, took some anti-inflammatory drugs.

On the 20th, Wang Jun found that the anal area grew a small lump, defecation is particularly painful. So he went to a hospital, the doctor found that his perianal has grown a small abscess, give him a cleaning, anti-inflammatory drip, the lump went down again.

After a few days, Wang Jun was afraid of infection after encountering water lumps, did not use water to clean, the pain is still there, but the lumps are back or grow, and do not know. By the 26th, the perianal pain intensified again, and he had no choice but to go to the Department of Anus and Intestines of the Affiliated Zhongda Hospital of Southeast University, where the lump not only did not recede, but instead grew to 7cm (in diameter) in size, and it was filled with pus.

Because the wound caused by the fish pointer was on the dentate gland inside the anus, the source of the abscess was also there, and the mass was large, which made it very inconvenient to operate. The doctor had to probe the source of the abscess and drain the pus before removing the abscessed tissue.

Wang Jun's attending physician, Chen Hao, said the abscess was caused by a fish splinter that cut the dentate gland in the anus and was not treated in a timely manner, which triggered an infection in the wound and led to a perianal abscess. The dentate gland is the secretion of defecation lubricating fluid tissue, is a moist environment, if there is a wound here, not cleaned well, it is easy to cause an abscess, good thing that the surgery is timely, if the abscess rupture after coming back, may be more trouble.

Fishbone pierced the aorta to death

A small fishbone, in the end, can harm people into what? A recent case shows that it can really kill you in a serious way.

Last December 12 at noon, 56-year-old Yang Cai (a pseudonym) and a few friends gathered to eat a few pieces of chub, after eating, he felt a little stuck in his throat, but did not care.

The next day, Yang Cai eat and swallow, or feel a little uncomfortable throat, but two days in a row, he can eat normally.

But by the evening of the 15th, Yang Cai felt that the fishbone is still not swallowed to the stomach, and there is a paroxysm of pain, decided to go to the hospital the next day to see.

Dec. 16, 10 a.m., Yang Cai's son accompanied him to Chongqing Long City Center Hospital to remove the fishbone. At that time, Dr. Jiang gave Yang Cai a gastroscopy and found that there was indeed a fishbone on the esophagus 25 centimeters away from the mouth. Because it was clearer through the gastroscope, he didn't do X-rays and CT and other tests to determine the size of the fishbone. The gastroscope will take less than 3 minutes to remove the fish head, the section of less than 1 centimeter long fish tail prick, there is a little blood at one end.

Since the fishbone was successfully removed, it should not be a big problem in general, and he was given some anti-inflammatory medicine and left the hospital.

But less than an hour after returning home, Yang Cai suddenly started vomiting blood from his mouth and nose, and lost consciousness for a while. Family members quickly sent him to the Hechuan District People's Hospital, where doctors injected him with an antiemetic needle and prescribed anti-inflammatory drugs and fluids.

Around 10:00 p.m., Yang Cai's condition further aggravated, despite the transfusion of 200 cc of blood, but still unconscious, around 11:00 p.m., Yang Cai was sent by ambulance to the intensive care unit of the Department of Thoracic and Cardiac Surgery of the First Affiliated Hospital of Chongqing Medical University, but the condition was not under control. 11:40 p.m., the amount of bleeding began to increase, from the previous blood gushing into the esophagus into the blood gushing into the chest cavity. Despite all efforts, he stopped breathing and beating at 00:50 a.m. the next morning.

The death certificate issued by the hospital reads, "Foreign body in the esophagus, aortic perforation hemorrhagic shock, respiratory arrest."

A fishbone cost 200,000 yuan

"I have also received a fishbone nearly killed the patient." Fan Xiugang, chief physician of cardiac surgery at Guangdong Provincial People's Hospital, still remembers it vividly.

In July 2005, Xiao Fang, a girl from Shunde, Guangdong province, suddenly developed a high fever and showed symptoms such as blood in her urine and stomach pain. Her family sent her to a local hospital, where she was X-rayed and found to have a "pseudo aortic aneurysm," which could lead to a life-threatening hemorrhage at any time.

Xiaofang was quickly sent to Guangdong Provincial People's Hospital and transferred to the pre-operative ICU observation room. After a number of blood, CT and other tests, found that her heart aortic blood vessels obviously have a foreign body sludge, and the arterial blood vessels have serious symptoms of infection, diagnosed as "pseudoaneurysm". A blood clot on the aorta is like a time bomb placed on a huge dam for the human body, once ruptured, it will lead to hemorrhage and death. Even more unfortunate, the examination also found that Xiaofang's esophagus also existed a serious part of the ulceration!

Because of the seriousness and complexity of the condition, the hospital organized a multi-disciplinary expert consultation and decided that the first step would be to first remove the infected arterial blood vessel and replace it with an artificial blood vessel. Then the esophagus was reconstructed.

When the aneurysm was analyzed after the operation, a sharp foreign object caught the doctor's attention, which turned out to be a small fishbone!

Then Xiaofang's mother remembered that a few days ago, Xiaofang ate fish stuck in her throat, when Xiaofang drank some vinegar, and swallowed a rice ball, after the pain was alleviated, Xiaofang thought that the fish splinter went down, so she did not pay too much attention.

I didn't realize that it was the swallowing of a few rice balls, not only did not bring the fish splinter to the stomach, but instead pierced the esophagus and out of the heart of the aortic blood vessels, resulting in chronic bleeding of the aortic blood vessels, cross-infections, and the formation of an aortic aneurysm. At the same time, due to the repeated puncture of the esophagus by the fishbone in the extrusion of the rice ball, resulting in severe ulceration and infection of the esophagus.

In the end, after two surgeries, hospitalized for more than 50 days and spending more than 200,000 yuan in medical expenses, finally retrieved Xiaofang's life.

Fishbone shadow misdiagnosed as lung cancer

Last November 21, when Mr. Zhang, who has been coughing up blood for many years, saw with his own eyes the CT film he had just made, he breathed a long sigh of relief. "I didn't realize it was a fishbone that was causing the problem, and my whole family thought I had cancer!"

The story began on New Year's Day 2005. Mr. Zhang is the Anhui Huangshan City, Shexian Wangcun Town, the family ate a reunion dinner on New Year's Day, Mr. Zhang ate some fish. There was a slight stinging sensation at the time, but then he ate some vegetables and the stinging sensation was no longer noticeable.

One day in October 2006, without warning, Mr. Zhang suddenly coughed up blood, about 500ml a day. Mr. Zhang went to Huangshan Shukang Hospital, where he was diagnosed with inflammation of the trachea and prescribed some anti-inflammatory drugs to stop the bleeding and went home. After a few days of taking the medicine, the coughing stopped and there was no discomfort, so there was no recurrence after that.

Until November 6 last year, Mr. Zhang again inexplicably coughed up blood, coughed up about 400 ml a day, and every time he breathed in, his lungs would feel a tingling sensation, and then the blood would gush out.

Seeing the seriousness of his condition, Mr. Zhang went to Huangshan People's Hospital to see a doctor. The doctor's initial diagnosis was lung cancer pending investigation. 55-year-old Mr. Zhang immediately felt a thunderbolt.

On November 9, the X-ray results showed that there was a high-density shadow in the lungs of 5×5cm in size, and the diagnosis of lung cancer was one step further. But oncologists combined other symptoms of lung cancer and felt that Mr. Zhang did not seem to have lung cancer.

To this end, Mr. Zhang underwent a CT examination, and a foreign object the size of a sewing needle in the black shadow on the CT film quickly dispelled the fear of "lung cancer".

This reminded Mr. Zhang of the fishbone pain he had ignored four years ago. At the time, it was brought downward by a vegetable, but unfortunately, it was instead inserted into the windpipe, puncturing it and then lodging in the lungs and causing an infection.

After two weeks of hospitalization, Mr. Zhang has now been discharged home, and a CT review has shown that the black shadows in his lungs have disappeared.

Eating fish must be dedicated (experts remind)

According to Luo Keqiang, chief physician of the otolaryngology department at the China-Japan Friendship Hospital, the clinic receives several cases of fish splinters in the throat every day, and when it comes to holidays, many people who don't usually eat much fish can't help but taste a little during the Spring Festival, leading to a significant increase in the number of cases.

Most patients are mild symptoms, to the outpatient clinic through the fiber laryngoscope, esophagoscopy, direct laryngoscopy, gastroscopy and other equipment can be found and remove the fish splinter, but because many people will be using their own folk "local methods", swallow rice balls, steamed bread, vegetables, drink vinegar and other methods to bring down the fish splinter, which is the main cause of fish splinter in the throat leading to serious illness, in fact, this is very undesirable! Because the fishbone by the rice ball, steamed bread extrusion, small fish spines are more likely to be deeper, and even puncture the esophagus, injury to the aorta, perianal and so on.
