2. Offline registration requirements
Applicants who have completed the online registration must pay close attention to the notice of the local health and family planning administrative department on the time and place for submission of candidate registration materials, and mail or submit the relevant materials to the relevant examination personnel designated by the local health and family planning administrative department within the specified time. Responsible agency.
Registration related materials (subject to the requirements of each province)
1. "2021 National Medical Equipment Users Business Ability Assessment Registration Form" (candidates can download and print by themselves after registration), which requires a personal signature and the signature and seal of the unit's human resources department.
2. A copy of your ID card.
3. A copy of the graduation certificate. The academic major must be consistent with the major applied for.
4. The unit shall issue a certificate proving that the applicant has been engaged in relevant work in the major applied for for more than two years.
5. Copy of practice registration certificate (provided by physicians).
Calculation of working years:
Only users of various types of medical equipment who have been engaged in the professional work for more than two years and have not yet obtained the "Large Medical Equipment Employment Qualification Certificate" or the "National Medical Equipment Users' Business Ability Assessment Certificate" can apply. Among them, applicants for physician majors Must hold a "Physician Practice Certificate". Apprenticeship and internship time prior to graduation shall not be counted.
