A brief discussion of how to implement emergency planning exercises to improve the first aid ability of nurses education paper
Abstract Purpose to explore the simulation of simulated first aid exercises to improve the first aid ability of emergency department nurses. Methods The emergency department nurses to carry out emergency plan learning and simulation simulation first aid drill, that is, the use of determining the content, develop a plan, develop a drill program, role configuration, on-site real-life simulation of first aid, the end of the drill to analyze and summarize. Results The emergency department nurses' basic skill operation scores and comprehensive ability scores in 20xx were significantly higher than those in 20xx after 4 drills (P all <0.01). Conclusion Simulated first aid drill is an effective method to comprehensively improve the comprehensive quality of emergency department nurses, first aid ability and overall coordination ability.

In order to improve the emergency department nurses' first-aid ability and the overall coordination ability, our emergency department combined with the actual situation of the department, from August 20xx to October 20xx, organized four simulated emergency preparedness drills for medical and nursing cooperation. After the training and drills, the first aid ability and comprehensive quality of nurses improved significantly, and is now reported as follows:
1 Objects and Methods
1.1 Objects There are 22 nurses in the Emergency Department, all of them are female, aged (25.68±3.12) years old, of which 1 is a bachelor's degree, 7 are junior colleges, 14 are junior colleges, 6 are supervisory nurses, 6 are nurse practitioners, 12 are nurses, and 6 are nurses. 6, nurse 12, nursing age (7.26±1.63) years. Since August 20xx ~ 20xx October organized four public **** unexpected events and acute food poisoning simulation of first aid drills, each time the whole department nurses are to participate in first aid drills, drills and the number of patients: a large number of firecracker explosions caused by injury to the patient (15 people), a large passenger car accident caused by serious multiple injuries to the patient (30 people) and acute food poisoning (organophosphorus pesticide poisoning) 35 people (students) . Types of events: patients with respiratory cardiac arrest, severe craniocerebral injury combined with closed chest injury, closed abdominal injury combined with shock, severe injury to the cervical spine and limbs, and sudden sudden death and coma during in-hospital escorted examination.
1.2 Methods
1.2.1 Establishment of the emergency plan exercise team The director of the emergency department and the head nurse act as the head and deputy head of the team, and the team members are composed of nurses and surgeons from the emergency department. The director of the department and the head nurse according to the characteristics of the emergency, the development of drill content, plan, develop a simulation of first aid specific program and staff role configuration, and finally by the Medical Department and the Department of Nursing organization and implementation.
1.2.2 Emergency preparedness drill implementation steps The first stage: study and discussion. After the first aid plan is determined, all nurses are organized to learn the theoretical knowledge and basic skills related to emergencies. Stage 2: Drill process. The head of the department and the head nurse preside over the drill, the whole drill process, project, content, real scene and clinical rescue consistent. By the staff play the patient's family, modeling people play patients or doctors, nurses play patients.
Method ①: emergency department nurses received a call for help signal quickly implement the emergency procedures, usually by two nurses, a doctor for a group with the ambulance quickly arrived at the scene on the spot rescue. Nurse A is responsible for maintaining a clear airway, establishing an effective intravenous channel, and administering medication; Nurse B measures BP and P and assists the doctor with chest compressions, electrocardiographic monitoring, defibrillation, and medication preparation and recording; and the doctor carries out first aid treatments according to the ABCD rule [1].
Method ②: Emergency nurse A answered the call for help, notify the doctor and nurse B quickly prepare the rescue supplies: such as emergency equipment in various departments, items positioned at the bedside, waiting for the patient. After the patient into the hospital according to method 1 of the personnel positioning quickly implement first aid treatment, under the command of the person in charge of the first aid team according to the plan of resuscitation. All the rescue process lasted 30~40min. during the emergency plan exercise, all the nurses who did not participate in the rescue participated in the on-site observation to increase the clinical experience and find the problems. The third stage: analysis and summarization. After the end of the exercise, all the staff to discuss and analyze, affirmation of achievements, identify problems, and finally, by the medical department and the director of the Department of Nursing on the medical and nursing aspects of the simulation exercise summary. Phase 4: Re-exercise. On the basis of the summary and analysis, by the nurses and doctors again exercise, emphasizing the integrity of the procedures, positioning and cooperation, to show a comprehensive, complete, standard emergency plan exercise process.

① Common basic operations: airway opening, ECG monitoring, venipuncture, defibrillation with electric shocks, unassisted cardiopulmonary resuscitation, and other five commonly used basic operations, the correct procedures in the process of first aid drills and the operation of the time of the assessment, in order to evaluate the degree of proficiency of a single operation: according to the score of 10 points ( Excellent), 8 points (good), 6 points (qualified), 4 points or less (unqualified) four grades to be evaluated.
② Comprehensive ability: clear division of labor, reasonable positioning, rapid movement, degree of accuracy, no repetitive action 5 comprehensive ability to score. Each item 5 points, a full score of 25 points, the higher the score, the stronger the overall ability.
1.2.4 Statistical methods The SPSS 11.0 statistical software package was used to process the data, and the t-test was used to measure the information.
2 Results

2.2 Comparison of the overall competence scores of the nurses in the two-year emergency drill The overall competence scores of the emergency department nurses who participated in the emergency drill in 20xx were significantly higher than those in 20xx, as shown in Table 2. 3 Discussion
3.1 The important role of the emergency preparedness drill The role of the simulation of the emergency preparedness drill is to provide a comprehensive understanding of the emergency preparedness of the nurses. The role of rehearsal is to think carefully about the rescue process, decisive response, tacit cooperation and coordination and relies on the integration of various roles and technologies into a behavioral model [1], so that the complexity and variability of the clinical condition can be reproduced, so as to improve the nurses' clinical emergency care level and ability.
3.2 Emergency planning exercises improve nurses' first aid skills, overall coordination and comprehensive ability Emergency department nurses are faced with acute onset, critical and complex patients every day, and their work is under great pressure and high risk, and there are relatively few opportunities for systematic learning and training [2]. Therefore, carrying out emergency plan drills enables emergency department nurses to systematically and completely experience and master basic theoretical knowledge and practical operational skills, fully embodies the intuition, practicality, effectiveness and controllability of drills [3], and comprehensively improves the comprehensive quality and overall coordination ability of emergency department nurses. The present results show that nurses participating in simulated first aid drills have their basic technical operation time shortened year by year with the `increase' in the number of drills, and their operational proficiency and comprehensive ability assessment scores improved year by year. As an effective training method, simulated first aid has been widely used in China [4].

3.3 Emergency plan drill to improve the success rate of resuscitation Emergency plan drill so that emergency department nurses systematically and comprehensively learn the emergency plan and simulate the whole process of first aid practice operation, through repeated operation, proficiency in first aid skills, improve the overall coordination ability, nurses to simulate the skills gained from training to apply to clinical work, so as to enhance the overall coordination ability, the nurse to simulate training, and the emergency department nurses to simulate first aid training. The nurse will apply the skills gained from the simulation training to the clinical work, thus improving the success rate of rescue.
3.4 Emergency preparedness drills and first aid skills training are interdependent and complementary to each other The Nursing Department and our department train emergency nurses three times a year in first aid skills and knowledge of first aid medication, etc. Compared with the emergency preparedness drills, the emergency preparedness drills are also complementary to each other. Compared with the emergency plan drill, the focus of first aid skill training is to master the first aid technology, which is the foundation of first aid work, while the focus of the emergency plan drill is the overall coordination of the first aid process and the mutual cooperation of the medical staff, which is the integration of various first aid techniques, therefore, the organic combination of the first aid skill training and the emergency plan drill is the interdependent and complementary aspects to improve the quality of the first aid, and the first aid skill training and the emergency plan drill are interdependent and complementary. After 2 years of emergency plan rehearsal, the basic theory, basic skills and overall comprehensive ability of emergency department nurses in clinical rescue work have been greatly improved. Therefore, the implementation of emergency planning exercises is a proven training method.
