Application and registration process of medical institution's practice license.

Legal analysis: 1. Units or individuals applying for the establishment of medical institutions shall submit an application report for the Practice License of Medical Institutions to the County Health Bureau, and fill in the Practice Registration Form of Medical Institutions, which shall include:

1, directory of legal representatives or principal responsible persons of medical institutions and heads of various departments, and copies of relevant qualification certificates, practice certificates and ID cards;

2, medical institutions housing property certificate or certificate of use;

3. Architectural design scheme of medical institutions;

4, capital verification certificate, asset evaluation report;

5. List of commonly used equipment and medicines.

Two, to apply for the practice registration of medical institutions, shall meet the following conditions:

(1) Having an approval letter for the establishment of a medical institution;

(2) meeting the basic standards of medical institutions;

(3) Having a proper name, organization and place;

(4) Having funds, facilities, equipment and professional health technicians suitable for its business;

(5) Having corresponding rules and regulations;

(6) Being able to bear civil liability independently.

Legal basis: Article 19 of the Regulations on the Administration of Medical Institutions The health administrative department of the local people's government at or above the county level shall, within 45 days from the date of accepting the application for practice registration, conduct an audit according to these Regulations and the Basic Standards for Medical Institutions. Those who pass the examination shall be registered and issued the Practice License of Medical Institution; If the audit is unqualified, the applicant shall be informed of the audit results in writing.
