Observation and nursing care of surgical patients using Clover positive pressure connector and indwelling needle

With the continuous updating of nursing work, Kelaifu connector indwelling needle infusion has been widely used in operating room, which is an advanced needle-free closed infusion technology with convenient and safe operation. In May-June, 2007, the patients who used intravenous infusion with Kelafu connector during the operation were investigated, and the next morning they went to the ward to observe whether there was blood return and blockage in the lumen of the trocar. Introduction is as follows.

1 observation result of physical method

1. 1 Subjects Patients who underwent surgery from May to June 2007, including 38 males and 34 females, aged 2-66 years. Peripheral superficial vein was used in all puncture sites. The indwelling trocar is 20Gy/22Gy produced by Braun Company in Germany, and the Kelaifu connector is CLC2000 and C 1000 produced by American ICU Medical Products Company.

1.2 method Before the infusion puncture, connect the female end of the Kelafu connector with the infusion tube, and then connect the male end of Kelafu connector with the indwelling trocar after venting. If it is confirmed that the indwelling needle is in the blood vessel before each infusion, turn on the water after connecting the connector, as shown in normal input. After infusion, unplug the infusion tube without washing it with heparin sodium or sealing it.

1.3 the next morning, I went to the ward to observe whether the indwelling trocar cavity of the patient had blood return and blockage. When infusing again, it is only necessary to disinfect the female end of the connector with 75% alcohol. When the nipple of the infusion tube is connected with the female head, insert it into the bottom and rotate it 90 degrees. Open the regulator and observe whether the infusion is smooth. When the patency rate reaches 40 ~ 60gtt/min, patency is effective again.

1.4 results Clinical observation showed that there were 1 cases of patients with type C 1000 who had blood return in the indwelling cannula cavity, but there was basically no blood return and blockage in type CLC2000. See table 1 for details.

2. 1 Reducing the risk factors of nurses' needle stick injury is the most important occupational factor leading to blood-borne diseases among health workers. With the increasing attention to safety in the world, the application of Clever has reduced the frequency of needle use and reduced the related acupuncture by 43%. In order to improve the safety of work, the operation is simple and there is no need to seal the pipe regularly. Every day, we only need to disinfect the interface with iodine to connect the infusion set, which greatly reduces the workload of nurses, improves the work efficiency, reduces the use of consumables such as heparin cap and empty needles for sealing infusion tubes, and reduces the economic burden of patients.

2.2 Reduce the occurrence of local phlebitis. The closed needle-free infusion system can avoid heparin blockage caused by repeated needling and heat source reaction caused by small particles entering the empty tube, which can reduce the chance of accidental infection during the operation of connecting heparin cap and inserting scalp needle, thus greatly reducing the occurrence of phlebitis.

2.3 Protecting patients' health not only reduces the workload of nurses, but also protects patients' health, especially some bleeding diseases with poor coagulation mechanism and patients after mechanical heart valve replacement.

2.4 Effectively reduce the blockage of indwelling needle. Positive pressure was used to reduce blood blockage, only complete blockage 1 case.

The positive pressure connector is convenient to use, simple to operate, prevents blood-borne infectious diseases, reduces catheter blockage, and does not need needle connection. 100% prevents needle stabbing. The unique interface design not only provides a simple access mode, but also reduces the possibility of pollution, increases the safety of tube sealing, not only greatly reduces the workload of nurses, leaves more time for patients, but also reduces the risk of nurses being contaminated by blood during operation. When the infusion connector is pulled out by positive pressure, the connector can be bounced up and closed naturally to prevent blood from flowing backwards.

In a word, the disinfection and connection operation of Clever needle-free infusion connector is simple, which relieves the pain for patients, ensures the smooth infusion, greatly reduces the workload of nursing staff and improves work efficiency.
