(II) Application method
1. On-site application. Applicants can apply for government information at the accepting institution of this organ and fill out the Application Form.
2. Apply in writing. After completing the Application Form, the applicant can apply by fax or letter. If applying by letter, the words "Application for Government Information Disclosure" should be marked in the lower left corner of the envelope. If an applicant applies for obtaining government information related to himself, he shall submit a written application in person with a valid identity document.
3. For online application, the applicant can fill in the application form in the information disclosure platform of Xinhua County Health Insurance Bureau.
4. Special application. Applicants who apply for government information related to themselves in terms of registration, social security, etc. shall hold valid identity documents and apply to their own organs.
