Pediatric Cardiac Surgery

The Department of Cardiac Surgery at Beijing Children's Hospital has been established for twenty-five years. Under the leadership of Professor Li Zhongzhi, the current president and the founder of the Department of Cardiac Surgery at BCH, the department has made unremitting efforts over the years to make the academic level of this department a leading one in the field of cardiac surgery for infants and young children in China. 1999, with the approval of the Beijing Municipal Bureau of Public Health, the Beijing Pediatric Congenital Heart Disease Treatment Center was established at BCH as part of the Cardiac Center, which includes the Department of Surgery of Congenital Heart Disease, the Department of Surgery, and the Department of Pediatrics. As part of the heart center, the center includes two parts, congenital heart disease surgical treatment and transcatheter interventional treatment. Thousands of various heart surgeries have been completed, and the survival rate has reached more than 98%, with the youngest age being 3 days old and weighing 3,800 g. In particular, the center is able to treat small infants with large ventricular interstices. Especially for small infants with large ventricular septal defects, tetralogy of Fallot and other critically ill children's surgery and postoperative monitoring is more unique, so that a large number of children with congenital heart disease due to timely treatment and save their lives. In recent years, focusing on complex congenital heart disease in newborns and small infants, a case of pulmonary artery atresia 8 days after birth was successfully treated by intracorporeal direct visualization under extracorporeal circulation, which is the first of its kind in China, and 70% of the operated patients had complex malformations and critical cases, including a large number of left-to-right shunt congenital heart disease secondary to severe pulmonary arterial hypertension, combined with pneumonia and heart failure, as well as complete transposition of the aorta, complete The Department of Cardiac Surgery has helped a large number of children with left-to-right shunt type congenital heart disease secondary to severe pulmonary hypertension, combined with pneumonia and heart failure, as well as complex conditions such as complete transposition of the great arteries, complete ectopic drainage of the pulmonary veins, complete atrioventricular channel and double outlet of the right ventricle. In addition, the Department of Cardiac Surgery has helped Changchun Children's Hospital, Jinan Military Hospital, Tianjin Children's Hospital, Dalian Children's Hospital and the Department of Cardiac Surgery of Shijiazhuang Children's Hospital to carry out surgical treatments for infants and young children with congenital heart disease, which has been highly praised by the local hospitals. Now there are hundreds of cardiac surgery cases every year, of which 70% are for infants under 2 years old and 50% are for infants under 1 year old.

Medical equipment conditions have also improved in recent years, with an investment of 8 million yuan to update and expand the beds and extracorporeal circulation equipment in the specialized cardiac surgery care unit. In the next few years, it is prepared to invest millions more to update more advanced ventilators, transesophageal ultrasound monitoring instruments and centralized multifunctional monitoring station to create better conditions for the treatment of infants with complex congenital heart malformations.

At present, the Department of Cardiothoracic Surgery has one full professor (chief physician), five associate chief physicians, three residents, and two doctoral and master's degree students. In recent years, the department has published dozens of papers, won one scientific research achievement award, trained seven master's degree students and two doctoral degree students, and carried out a large number of international academic exchanges. Several cardiac surgeons and nurses have gone to the U.S.A., Japan, Germany, Italy, Russia and other countries to visit or study, and invited a number of overseas specialists to the hospital to conduct academic lectures and surgeries. We have successfully held the national study class on the progress of diagnosis and treatment of pediatric congenital heart disease, and applied for the right to hold the 5th THIC International Pediatric Cardiology Symposium, which is now under active preparation.

Interventional treatment of congenital heart disease and cardiovascular imaging

Interventional treatment of heart disease is an alternative to cardiac surgery, which can be accomplished without the need to open the chest, and can be accomplished by percutaneous percutaneous puncture of a blood vessel with special catheters that deliver the appropriate materials. It eliminates the pain and trauma of surgery, provides fast recovery, is safe and reliable, and is one of the major advances in the treatment of pediatric heart disease in recent years. Since 1989, our center has been carrying out transcatheter interventional therapy, applying balloons, spring coils, blockers and stents to treat a variety of congenital heart diseases, including valvular stenosis, arteriovenous fistulas, arterial ductus arteriosus, atrial septal defect, ventricular septal defect, etc., and all of them have achieved satisfactory results. In addition, we have experience in treating anomalies such as coronary artery stenosis caused by pediatric Kawasaki disease.

Transcatheter cardiovascular angiography is an invasive diagnostic method performed when non-invasive diagnostic techniques fail to make a completely clear diagnosis. After high pressure injection of contrast medium through a special cardiac catheter via percutaneous perforation of blood vessels in a special part of the heart or large blood vessels, the anatomical structure and blood flow of the heart or large blood vessels can be further understood, and hemodynamic changes of the heart can be understood by measuring the blood oxygen in different parts of the heart.

At present, the center has one double C-arm cardiovascular angiography machine with 1250 mA X-ray and corresponding cardiac catheterization equipment attending the center, one chief physician, two deputy chief physicians and one attending physician.

Cardiac electrophysiology and arrhythmia interventional therapy

Interventional cardiac electrophysiology examination and radiofrequency ablation is an interventional procedure that goes through the femoral vein puncture route, delivering probe electrodes or radiofrequency ablation catheters, and arriving along the blood vessels to the cardiac cavity.

Radiofrequency ablation is the only way to eradicate supraventricular tachycardia. It has gradually matured since its introduction in the 1980s. The current overall success rate is 97%. It has the advantages of quick effect, small trauma and relatively low cost. Previously, supraventricular tachycardia was treated only with drugs or open heart surgery. Drugs are taken daily, which is time-consuming and requires regular follow-up because of side effects. The medications were time-consuming and required regular follow-up due to side effects, and the medications could be changed repeatedly due to poor results, causing great inconvenience and even danger to the patient. Open-heart surgery is highly invasive and difficult to promote.

Children have significant anatomical differences from adults. Such as children's heart cavity is narrow, thin wall blood vessels, valves tender and easy to damage, the reproductive organs are not yet mature, very sensitive to X-rays, very easy to damage. This determines that pediatric patients must be operated by a pediatrician. The anesthesia, X-ray protection, catheter operation, medication, and supervision of the surgery are obviously different from those of adult patients.

BCH has introduced the most advanced radiofrequency ablation equipment in China and redesigned the catheterization room to successfully carry out this kind of surgery. At the same time, Beijing Children's Hospital has a complete range of specialties, including cardiac and thoracic surgery, ICU, anesthesiology and cardiology with strong technical strength. From the preoperative preparation, intraoperative radiation protection, catheter operation, postoperative monitoring until the discharge of the children are all well guaranteed. As the largest children's hospital in China, it is the most suitable for the majority of pediatric patients to undergo such treatment.

Indications for electrophysiologic examination and radiofrequency ablation:

Pre-excitation syndrome

Suspected syncope of cardiovascular origin (about 25% of syncope)

All kinds of tachycardia (including: atrial, ventricular, paraventricular and intersectional)

Placement of atrioventricular block

In case of bradyarrhythmias ( sinus bradycardia, severe sinus block, sinus arrest, severe AV block) temporary or permanent pacemakers can also be installed to save the child's life and improve quality of life.

The Department of Echocardiography has a history of 20 years, and there are two chief physicians, two deputy chief physicians, and two attending physicians with formal training in echocardiography. The number of echocardiograms per year is more than 10,000, which includes all types of congenital and acquired cardiac disorders, and there is a wealth of experience in diagnosis of complex malformations of the heart, and in diagnosis of pediatric cardiomyopathies, myocardial diseases, endocardial elastosis, and Kawasaki's disease. Fibroproliferative disease, Kawasaki disease, endocarditis, pericardial disease, rheumatic heart disease and other cardiology diseases. At present, we are equipped with 5 sets of the world's most advanced echocardiographs. In addition to routine transthoracic echocardiograms, we also perform pediatric transesophageal echocardiograms, perioperative transesophageal or epicardial echocardiograms, drug-loaded echocardiograms, cardiac ultrasound acoustic imaging, and three-dimensional echocardiograms, which have greatly enhanced the level of diagnosis and adjunctive treatment of heart disease.
