4. Activity content

(1) Continue to promote the "Three Basics and Three Stricts" training.

On November 9, all medical, pharmaceutical and technical staff of the hospital will be arranged to take the "Three Basics" knowledge examination to ensure the basics, promote quality and improve the level.

(2) Medical record evaluation and exhibition evaluation.

Time: November 5th---November 30th

Medical record inspection and evaluation method:

1. Based on the "Municipal Third People's Hospital Medical Record Inspection" "Grade Scale" is mainly used, and individual veto items are added.

2. The form (item) of medical records in this exhibition and review accounts for 40 points (such as homepage, disease course recording time, stage summary, rescue and death records, etc.), and the content accounts for 60 points (such as current history, diagnosis basis, etc.) , differential diagnosis, third-level ward rounds, treatment and surgical records, various protocols, etc.).

3. In this medical record inspection and evaluation activity, discharge medical records from June to October were selected. Each department provides 5 discharge medical records. The medical records provided by each specialty should be the top five diseases in that specialty. Medical records entering the clinical path are the first choice.

4. The hospital medical record quality management committee shall organize each member to conduct the work in accordance with the Ministry of Health's "Basic Standards for Writing Medical Records" and "Department of Health's Inpatient Medical Record Writing Quality Assessment Standards" ("Department of Health's Basic Standards for Writing Medical Records") Evaluation.

(3) Standardized physical examination competition among doctors at all levels.

Participants: all doctors

1. The judges are hired from outside hospitals and use five judges. Equal share system.

2. Competitors will randomly select one item from the five items of physical examination, heart examination, chest and lung examination, abdominal examination, head and neck examination, and back and limb examination to compete.

3. Two players will compete at the same time in each game. The competition time is 15 minutes. Points will be deducted for overtime.

(4) Carry out in-depth high-quality nursing service projects with the characteristics of our hospital.

1. First, arrange a special lecture on high-quality nursing services, and invite nursing teachers to give lectures in the first week of November. The purpose is to standardize nursing behavior, gain an in-depth understanding of the progress of current nursing work, and better implement high-quality nursing services. Nursing services.

2. Arrange head nurses and nursing backbones of each department to go to high-quality nursing wards to observe the nursing services provided by nursing staff to patients, such as: admission care, management of critically ill patients by responsible nurses during hospitalization; discharge care, etc. The time is scheduled for the first week.

(5) Strictly focus on the tracking, analysis, feedback and other management of adverse nursing events to reduce the chance of occurrence.

In order to achieve the purpose of warning and educating nursing staff and reducing the occurrence of adverse events, the Nursing Department will organize a meeting for all nursing staff in the second week of November, and invite senior and experienced head nurses to take turns to review the department’s past practices. Carry out case analysis and summary of adverse nursing events that occur, find out the attribution, and provide warning education at meetings.

(6) Conduct theoretical and operational assessments for all nursing staff in the hospital based on the specialty characteristics of different departments.

The standardized nursing behavior theory and operation test will be held in the third week of November. The content of the theoretical test will be based on the characteristics of the specialty. The operations are divided into two parts: aseptic technique operation and reception of newly admitted patients.

(7) Prescription quality writing evaluation. Experts were selected from the expert database to comment on the prescriptions from June to October, and each person selected 20 prescriptions for selection.

(8) Exchange symposium on special treatment of antimicrobial drugs, allowing departments with higher rates of pathogenic testing of inpatients to exchange experiences. Evaluation of model doctors for perioperative prophylactic use of antibacterial drugs, and 10 medical records of each surgeon from June to October were selected and commented.

(9) Exchange meeting on experience in reporting adverse drug reactions, and conduct follow-up and follow-up on individual adverse drug reaction events. Reward individuals with high report quality.
