What formalities are needed for special diseases

Materials needed to apply for special diseases:

(1) Application Form for Outpatient Special Diseases and Treatment Items filled out by a physician with the title of attending (including attending) or above in the relevant specialty of the designated hospital of the second level or above (or specialist), and examined and stamped by the Medical Insurance Office;

(2) Relevant medical records (information on the medical treatment for the last three months or more);

(3) Relevant examination and test reports and discharge summary (copy of original). Different special diseases need to provide the corresponding examination and test reports as required.

Legal basis

Article 3 of the "Regulations on the Administration of Safety Training and Examination of Special Operators" stipulates that the special operators are employees directly engaged in special operations. Article 3 also stipulates that special operations, is prone to accidents, the operator himself, the safety and health of others and the safety of equipment and facilities may cause significant harm to the operation. The scope of special operations is stipulated in the Special Operations Catalog. In the "catalog of special operations" stipulates that the scope of special include:

1, electrician work.

2, welding and thermal cutting operations.

3, work at height. Refers to specialized or often in the fall height of 2 meters above the base surface and the possibility of falling from a high place of operation.

4, refrigeration and air-conditioning operations. Refers to large and medium-sized refrigeration and air-conditioning equipment operation, installation and repair operations.

5, coal mine safety operations.

6, metal non-metallic mine safety operations.

7, oil and gas safety operations.

8, metallurgical (non-ferrous) production safety operations.

9, hazardous chemical safety operations. Refers to the operation of hazardous chemical processes and chemical automation control instrumentation installation, repair, maintenance operations.

10, fireworks safety operations.
