I. Conditions for declaration:
Participants who comply with the regulations on family planning, are insured by their units and have paid contributions continuously for more than six months without any arrears.
Second, the declaration procedure:
(1) Maternity, family planning surgery registration
1, female workers who are pregnant or need family planning surgery, the unit for their registration procedures; and after consulting the will of the person, for the selection of the first maternity examination institutions, mid-late stage maternity examination institutions and inpatient maternity medical institutions or surgical medical institutions;
2, the handling of Maternity registration shall be made by filling in five copies of "Wuhan Maternity Insurance Maternity Medical Registration Form" (hereinafter referred to as "Form 1"), stamped with the official seal of the organization, and accompanied by the original and photocopies of "Maternity Service Certificate", ID card, and medical insurance card; termination of pregnancy shall be made by filling in "Form 1" and "Form 1". For registration of termination of pregnancy, three copies of "Form 1" shall be filled in, and the official seal of the unit shall be affixed; for registration of family planning surgery, two copies of "Wuhan Municipal Maternity Insurance Family Planning Surgery Registration Form" (hereinafter referred to as "Form 2") shall be filled in, and the official seal of the unit shall be affixed, and the original and photocopies of the marriage certificate, medical insurance card and ID card shall be attached. In the case of ring removal and oviductal (sperm) reversal surgery, a relevant certificate from the family planning management department and a diagnostic opinion from the attending doctor of the designated hospital must also be produced. The above services are handled at Window No. 5 on the second floor;
3. After examination and approval by the staff, the staff will affix a special audit seal on "Form 1" or "Form 2" and register the machine; one copy of the returned form will be retained by the unit, and the rest will be handed over to the medical institution by the female worker herself;
4. If the female worker is a permanent resident, she is also required to fill in two copies of "Wuhan Maternity Insurance Application Form for Medical Treatment for Permanent Residents" (hereinafter referred to as "Form 3"), and attach the unit's certificates to the medical institutions and social security agencies to send the seal of the resident, and then the unit's staff will report to the window No. ⑤ on the second floor for preliminary examination and report to the Municipal Workers' Compensation and Maternity Insurance Center for approval. Reported to the Municipal Workers' Compensation and Maternity Insurance Center for approval.
(2) Maternity Allowance and Nursing Allowance Claims
After a female worker has given birth or suffered a miscarriage, the unit will go through the procedures of applying for maternity allowance for her.
1. The unit shall fill in two copies of "Wuhan Maternity Allowance and Nursing Leave Allowance Declaration and Audit Form" (Form 6), stamp the official seal of the unit, and attach the approved "Form 1" (one copy), the hospital discharge summary, and a copy of the birth certificate, and report it to Window No. ⑤ on the second floor for the accounting formalities;
2. The staff shall verify the information and then process the application for maternity allowance for them. After verifying the information, the staff will go through the procedure of accounting for the maternity allowance, affix the official seal and report it to the Municipal Industrial Injury and Maternity Insurance Center; the unit will go to the Municipal Industrial Injury and Maternity Insurance Center for the procedure of receiving the maternity allowance in the following month.
After the birth of a male employee's spouse, the unit has to go through the procedure of applying for the nursing leave allowance for him, and the process is the same as that of applying for the maternity allowance, with the attached documents being the original and copies of birth certificates and one-child certificates;
If it is a unit fully funded by the treasury, the maternity allowance and the nursing leave allowance for the employee will be settled between the Center of Municipal Industrial Injury and Maternity Insurance and the treasury, and will not be claimed by the unit.
(3) Reimbursement of Maternity Medical Expenses
Maternity medical expenses incurred by long-staying employees in foreign places that are advanced by individuals at the place where they are sent shall be settled by the unit within 30 days after the medical treatment has been finalized;
1. The unit fills in two copies of the "Wuhan Municipal Maternity Insurance Cash Settlement Declaration and Audit Sheet" (Form 5), stamps the official seal of the unit, and attaches the approved "Form 1" (three copies), "Form 3" (one copy), maternity invoice, hospitalization invoice, relevant checklists, diagnostic certificates, summary list of medical expenses (the above are originals), and the first page of the medical case, the surgical record sheet, and the discharge summary (the above are photocopies) are submitted to the 2nd Floor Window ⑤ for audit and settlement procedures;
2. After verifying the information, the staff will handle the audit and settlement procedures of medical expenses for them, and then report them to the Municipal Workers' Compensation and Maternity Insurance Center with official seal, and then transfer the reimbursement expenses to their own medical insurance cards after reviewing.
Third, the declaration time:
1, maternity registration time: from the 1st to the 20th of each month (except holidays, the same below);
2, maternity allowance, nursing allowance claim time: before the 10th of each month after the employee's delivery or abortion (induced);
3, the time of reimbursement of maternity medical expenses: within 30 days after the medical treatment is finalized.
