There is often spontaneous nystagmus, postural test nystagmus lasts more than 30 seconds, no latency, no fatigue, CT and MRI can further confirm the diagnosis.

2. Vestibular neuritis

Sudden onset, which may last for several days, hypothermia on cold and heat test, history of upper respiratory tract infection, mild leukocytosis on blood.

3. Cervical spondylosis

Cervical spondylolisthesis, can compress the vertebral artery or stimulate the cervical sympathetic nerves and cause spasm of the vertebral artery, so that the blood supply to the vestibule is insufficient, most often in adults over 40 years of age, the onset of vertigo is related to a specific head and neck position. It is often combined with other symptoms of insufficient blood supply to the vertebral basilar artery, such as headache, visual disturbances and numbness of the upper limbs. Cervical spine radiographs show osteophytes or other deformities in the 4th to 6th cervical vertebrae.
