Do I need genetic testing before taking PD1 or PDL1?

Genetic testing related to PD1 and PDL1 is recommended before treatment. If the genetic test shows positive, it means that the patient is well suited for PD1 immunotherapy.

Before applying PD-1 or PDL1 therapy, a combination of multi-gene tests can be done to look for positive or negative regulator genes of the therapy to predict the therapeutic effect, adverse effects, and also the expression level of PD-1 and the level of tumor mutation load should be tested. Some studies have shown that the higher the degree of PD-L1 expression, the better the efficacy of immunotherapy, and PD-L1 (programmed death receptor) has been approved by the FDA as one of the biomarkers for predicting the efficacy of immune checkpoint inhibitor drugs. Results can be obtained by immunohistochemical staining of cancer tissue or by blood testing for circulating tumor cells (CTC). Detection of PD-1 and PD-L1 prior to using PD-1 antibodies is useful in predicting prognosis, but the absence of PD-1 or PD-L1 expression does not mean that the use of PD-1 antibodies is ineffective, as there may be false positives or false negatives in genetic testing. Without knowing the exact condition of the patient and what treatment has been administered, the specific treatment plan needs to be comprehensively formulated by the doctor in charge, so if you have any questions, please feel free to communicate with the doctor in charge and listen to the doctor's advice.
