Medical institutions applying for the establishment of intravenous drug dispensing centers shall submit the following information to the administrative department of health:

2, license to practice as a medical institution (copy);

3, copy of academic and professional title certificates of the person in charge of the intravenous drug dispensing center and the personnel in charge of the review of the suitability of the intravenous medication prescription or prescription

4, intravenous drug dispensing related work system and norms (copy);

5, in accordance with the "norms" requirements, other materials that need to be explained.

The health administrative department that receives the application shall complete the audit, acceptance and approval within 30 working days. Qualified by the audit, agreed to set up intravenous drug dispensing center, should be noted in the applicant medical institution's copy of the license to practice in the remarks column.
