The procedures required for handling the disability certificate are as follows:
1. Application: The applicant holds a resident ID card, a household registration book and three recent two-inch bareheaded color photos on a white background, and applies to the county-level disabled persons' Federation where the household registration is located;
2, assessment, county-level Disabled Persons' Federation to confirm;
3. Review, approve and designate disability assessment agencies;
4. issue a disability certificate.
If the category or level of disability changes, the disability appraisal can be re-conducted in the designated institution upon my own application and with the consent of the Disabled Persons' Federation. According to the evaluation results, the Federation of the Disabled is approved to renew the disabled card, and the relevant information in the basic database of the disabled population is changed.
If the disabled person's disability status no longer meets the disability standard or dies, the CDPF issuing certificate shall cancel the disabled person's card in time; Disabled persons or guardians of intellectual, mental and minor disabled persons who request the cancellation of the disabled card shall submit corresponding identification materials and a written application, and the issuing disabled persons' federation may withdraw the disabled card and cancel the relevant information in the basic database of the disabled population. After the cancellation of the disabled card, it shall not be reapplied within one year. The determination of the change of disability status of the disabled shall be based on the conclusion of disability assessment made by the designated institution.
"Measures for the Administration of Disabled Persons' Cards in People's Republic of China (PRC)" Article 7 The county-level Federation of Disabled Persons shall be responsible for the acceptance and issuance of disabled persons' cards.
According to the conclusion of disability classification and disability rating made by the hospitals or professional institutions designated by the Provincial Health and Family Planning Commission and the Disabled Persons' Federation (hereinafter referred to as the designated institutions), the county-level disabled persons' Federation issues disabled persons' cards and is responsible for the identification of the original file management.
Provincial disabled persons' federations and municipal disabled persons' federations shall guide, supervise and inspect the issuance, use and management of disabled persons' cards. The Provincial Disabled Persons' Federation and the Health and Family Planning Commission set up an expert committee on disability assessment, which is responsible for accepting disputes over disability assessment.
Twenty-seventh in the issuance and management of disabled persons' cards, in any of the following circumstances, the responsibility of the relevant personnel shall be seriously investigated and disciplinary action shall be given according to the relevant provisions. Those suspected of violating the law and committing crimes shall be transferred to judicial organs for handling.
Disability assessment is fraudulent;
(two) in violation of the provisions of the disabled card;
(three) making things difficult for the disabled and deliberately delaying the handling;
(four) the disclosure of personal information of the disabled has serious consequences.
