(a) gradually establish health records. Establishment of urban and rural residents health records should be in the county (city, district) health administrative departments under the unified leadership of the community health service centers, community health service stations and township health centers, village health offices and other urban and rural primary health care institutions specifically responsible for. Health records are established for residents in a variety of ways, including through the provision of basic national public **** health services, daily outpatient clinics, health checkups, and home visits by medical staff, and records are made accordingly based on the provision of services. The information in the health records should be complete, truthful and accurate. Other medical and health institutions should cooperate in the supplementation and improvement of health records. (ii) Effective use of health records. Health records shall be stored uniformly in urban and rural primary health care institutions. When establishing and using health records for residents, urban and rural primary health care personnel shall comply with the Practicing Physicians Law, Regulations on the Administration of the Practice of Rural Doctors, and other relevant laws and regulations. Primary medical and healthcare organizations shall establish a system for the retrieval, inspection, recording and storage of residents' health records, and clearly define the persons responsible for the management of residents' health records, so as to ensure that residents' health records are conveniently used and kept for safekeeping. County health administrative departments should implement the establishment of health records of institutions, personnel, funding and facilities and other safeguards and strengthen the establishment of health records supervision and management.

Residents' health records, once established, shall be kept for the lifetime of the residents. To comply with the file security system, shall not cause damage to the health records, lost, shall not be unauthorized disclosure of personal information in the health records of residents and private information related to the health of residents.
