Article XI of the application for food business license, shall meet the following conditions:
(a) with the operation of food varieties, the number of food raw materials handling and food processing, sales, storage and other places, to maintain the environment of the place is clean and tidy, and with toxic, hazardous places and other sources of pollution to maintain the prescribed distance;
(b) with the operation of food varieties, the number of business equipment or facilities, there are appropriate disinfection, disinfection, and other sources of pollution;
(c) the operation of food varieties, the number of food varieties, the number of business facilities. (B) has with the operation of food varieties, the number of appropriate business equipment or facilities, there are appropriate disinfection, dressing, washing, lighting, ventilation, antiseptic, dust, fly, rodent, insect, washing, as well as the treatment of wastewater, storage of garbage and waste equipment or facilities;
(C) a full-time or part-time food safety management personnel and to ensure that the rules and regulations of food safety;
(D) Have a reasonable layout of equipment and process flow, to prevent the food to be processed and direct access to food, raw materials and finished products cross-contamination, to avoid food contact with toxic substances, unclean substances;
(e) laws and regulations provide for other conditions.
Article XII of the application for food business license shall be submitted to the local food and drug supervision and management department at or above the county level where the applicant is located to submit the following materials:
(a) application for food business license;
(b) business license or other copies of documents certifying the qualifications of the main body;
(c) the layout of major equipment and facilities compatible with the food business, Operating procedures and other documents;
(d) food safety self-inspection, health management of employees, purchase and inspection records, food safety accidents and other regulations to ensure food safety.
The use of vending equipment engaged in the sale of food, the applicant shall also submit vending equipment, product certification, the specific placement of the location, the operator's name, residence, contact information, food business license and other publicity methods.
The applicant entrusted others to apply for food business license, the agent shall submit a power of attorney and the agent's identity documents.
Article XIII of the applicant shall truthfully submit to the food and drug supervision and management department and reflect the true situation, the authenticity of the application materials are responsible for, and in the application and other materials on the signature or seal.
