1. Application form for construction project planning completion acceptance certificate (parallel acceptance project) 1 copy (original);
2. One copy (original) of the completed drawing of the construction project (for the construction project, only the completed floor plans, elevations, sections and the completed general plan are submitted);
3. Prepared by a surveying and mapping unit with corresponding qualifications 2 copies of the house completion measurement report (including electronic documents) (original);
4. 1:500 completed measured topographic map of the construction project measured and drawn by a surveying and mapping unit with corresponding qualifications (with inspection line receipt attached, including Electronic documents) 2 copies (original);
5. For municipal pipeline projects, submit 2 copies (original) of pipeline completion drawings and surveying and mapping data (including electronic documents) that have passed the acceptance of the Municipal Surveying and Mapping Product Quality Supervision Station.
(3) Materials required by the public security and fire department (one copy each)
1. Information required for civil engineering acceptance declaration
⑴ Construction project fire acceptance declaration form ;
⑵Construction project preliminary design review opinion letter (copy);
⑶Project completion acceptance report;
⑷Construction permit issued by the construction administrative department Certificate (copy);
⑸ Fire protection product list and quality certification documents;
⑹ Fire protection facilities and electrical fire protection technology testing certification documents;
⑺ The legal identity certificate and qualification level certification document (copy) of the fire protection facility construction, project supervision, and testing unit should be marked "This document is verified to be correct with the original" and stamped with the official seal of the unit;
⑻ Yes Certification documents and factory certificates that building components and building materials required for fire protection performance meet national standards or industry standards;
⑼Complete drawings related to fire protection acceptance (general plan, standard floor plan and non-standard floor plan , elevations, sections, plans and system diagrams of various fire protection facilities) and concealed engineering records;

2. Fire protection acceptance stage of decoration project
⑴ Construction project fire protection acceptance declaration form;
⑵ Construction project decoration design review opinion letter (copy);
⑶ Letter of acceptance of fire protection for the main building (copy);
⑷ Project completion acceptance report;
⑸ List of fire protection products and quality certification documents;
⑹ Documents proving that the electrical fire protection technology has passed the test. If there are modern fire protection facilities, the documents that have passed the test of the fire protection facilities should be provided;

(4) Materials required by the environmental protection department
1. Environmental protection pre-acceptance (trial production):
(1) Environmental protection pre-acceptance of Chongqing construction projects ( Trial production) application form 3 copies (original).
2. Environmental protection acceptance:
(1) 5 copies (original) of the environmental protection acceptance application form for completion of the construction project (divided into 4 forms according to the specific conditions of the construction project);
① Application form for environmental protection acceptance and pollutant discharge upon completion of construction projects (applicable to construction projects focusing on pollutant discharge);
② Application registration card for environmental protection acceptance upon completion of construction projects (Applicable to construction projects that mainly focus on ecological impact and fill out the environmental impact registration form);
③ Application form for environmental protection acceptance of completed construction projects (applicable to construction projects that mainly focus on ecological impact and prepare an environmental impact report form) Construction projects);
④ Application report for environmental protection acceptance of completed construction projects (applicable to construction projects that focus on ecological impact and prepare environmental impact reports).
(2) Two copies (original) of the completed environmental protection acceptance monitoring report or acceptance investigation report of the construction project prepared by a qualified monitoring agency or environmental assessment agency (for construction projects that fill in the completed environmental protection acceptance application registration card, The applicant does not provide this application material).
