The state has implemented a licensing system for animal diagnosis and treatment. Institutions engaged in animal diagnostic and treatment activities should obtain animal diagnostic and treatment license, and within the scope of the prescribed diagnostic and treatment activities to carry out animal diagnostic and treatment activities.
Conditions
Application for the establishment of animal diagnostic and treatment institutions, should have the following conditions:
(a) a fixed animal diagnostic and treatment places, and animal diagnostic and treatment places in line with the provisions of the veterinary department of the people's governments of the provinces, autonomous regions and municipalities directly under the central government;
(b) animal diagnostic and treatment places from the location of animal and poultry farms, slaughter and processing, animal trading places not less than 200 meters;
(c) animal diagnostic and treatment places away from the farm, the slaughtering and processing, animal trading places. Trading places not less than 200 meters;
(C) animal diagnostic and treatment facilities with independent entrances and exits, entrances and exits shall not be located in residential buildings or courtyards, and shall not be used with other users of the same building *** access;
(D) has a reasonable layout of the diagnostic and treatment rooms, operating rooms, pharmacies and other facilities;
(E) with diagnostic, surgical, disinfection, cold storage, Routine laboratory, sewage treatment and other instruments and equipment;
(F) has more than one person who has obtained the qualification certificate of licensed veterinary surgeon;
(G) has a perfect diagnosis and treatment services, outbreak reporting, sanitation and disinfection, veterinary prescription, drugs and harmless disposal and other management systems.
Animal diagnostic and treatment institutions engaged in animal cranial, thoracic and abdominal surgery, in addition to the conditions set out in Article 5 of these Measures, it should also have the following conditions:
(a) with an operating table, X-ray machine or ultrasound and other instruments and equipment;
(b) with more than three obtaining the certificate of veterinary surgeon qualification.
Application
To establish an animal clinic, an application shall be made to the licensing authority where the animal clinic is located and the following materials shall be submitted:
(1) application form for an animal clinic license;
(2) geographic location map of the animal clinic, indoor floor plan and the layout of each functional area;
(3) the animal clinic Proof of the right to use;
(4) legal representative (person in charge) identification;
(5) the original and a copy of the certificate of qualification of practicing veterinarians;
(6) a list of facilities and equipment;
(7) the text of the management system;
(8) licensed veterinarians and health certification of service personnel.
If the application materials are incomplete or do not meet the prescribed conditions, the licensing authority shall inform the applicant of the contents to be corrected in one go within five working days from the date of receipt of the application materials.
Animal clinics should use a standardized name, do not have the ability to engage in animal cranial, thoracic and abdominal surgery, shall not use the name "animal hospital.
Animal diagnostic and treatment institutions should be pre-approved by the administrative authorities for industry and commerce.
Audit
After accepting the application, the licensing authority shall, within 20 working days to complete the application materials and review of the animal clinic site inspection.
Issuance of license
Compliance with the prescribed conditions, the licensing authority shall issue the applicant an animal diagnosis and treatment license; does not meet the conditions, notify the applicant in writing, and explain the reasons.
Specializing in aquatic animal disease diagnosis and treatment, the licensing authority in the issuance of animal diagnosis and treatment license, shall seek the views of the fisheries administrative department at the same level.
Animal diagnosis and treatment license shall contain the name of the diagnosis and treatment institutions, diagnosis and treatment of the scope of activities, practice location and legal representative (responsible person) and other matters.

Industrial and commercial registration
Applicants with animal diagnosis and treatment license to the animal diagnosis and treatment place where the administrative department for industry and commerce for registration procedures.
Animal diagnosis and treatment institutions to set up branches, should be a separate animal diagnosis and treatment license.
Animal diagnosis and treatment institutions to change the name or legal representative (person in charge), should be in the industrial and commercial change of registration procedures within 15 working days, to the original licensing authority to apply for change procedures.
Animal diagnosis and treatment institutions to change the place of practice, the scope of diagnostic and therapeutic activities, should be required to re-apply for animal diagnosis and treatment of licensing procedures, apply for the replacement of animal diagnosis and treatment of licenses, and according to law for industrial and commercial changes in the registration formalities.
