TSG Z0005-2007 Detailed Rules for Evaluation of Manufacturing, Installation, Modification and Maintenance License of Special Equipment
Article 10 During on-site evaluation, the applicant shall provide the following information to the evaluation team:
(a) the basic information of the applicant;
(2) Documents (originals) and organization code certificates (originals) registered with the local government according to law;
(3) The special equipment license (original) held by the replacement applicant and the list of special equipment manufacturing products (installation, modification and maintenance equipment) during the license holding period;
(four) the quality assurance manual of special equipment and its related program documents and operation (process) documents;
(5) List of quality assurance engineers and persons in charge of quality control system, letters of appointment, employment contracts, payroll, relevant insurance certificates, ID cards, professional title certificates and academic certificates;
(six) the list of engineering and technical personnel and special equipment operators (welding, nondestructive testing) and their employment contracts, payroll, relevant insurance certificates, identity cards, professional titles, academic qualifications and certificates of special equipment operators (original);
(seven) equipment, tooling, instruments, appliances, inspection and test devices and other ledgers;
(eight) calibration ledger and calibration records of inspection and test devices;
