If you are hospitalized in other places, you need the local hospital in your hometown to issue the transfer procedures for transferring to xx hospital in other places; In case of emergency in other places, you should register with the local cooperative medical management department in your hometown within three days of hospitalization (policies vary from place to place).
Need to go back to my hometown for reimbursement.
The procedures required for reimbursement are as follows:
1, copy of inpatient medical records (only "discharge summary" is needed in some areas)
2, the total cost list
3. Hospitalization receipt (original)
4. Diagnosis certificate
5. ID card and household registration book
6, cooperative medical certificate (or medical card)
7. Transfer program or certificate
For the first four items, ask the doctor when you leave the hospital.
