Grade 0: event stopped before execution;
Grade I: event occurred and was executed, but did not cause harm;
Grade II: minor harm with no change in vital signs, requiring clinical observation and minor treatment;
Grade III: moderate harm with some changes in vital signs, requiring further clinical observation and simple treatment;
Grade IV: severe injury, significant changes in vital signs, requiring elevated level of care and emergency treatment;
Grade V: permanent loss of function;
Grade VI: death.
