Medical institution application process

Units or individuals applying for the establishment of medical institutions shall submit an application report for the Practice License of Medical Institutions to the County Health Bureau and fill in the Practice Registration Form of Medical Institutions. The submission method is as follows: 1 The legal representative or principal responsible person of the medical institution, the list of responsible persons of various departments and the copies of relevant qualification certificates, practice certificates and identity cards; 2. Proof of ownership or use of medical institutions; 3. Architectural design scheme of medical institutions; 4. Capital verification certificate and asset evaluation report; 5. List of commonly used equipment and medicines. County Health Bureau shall, within 45 days from the date of accepting the application, review the application materials, send personnel to the site for inspection and approval, and the undertaker or handler shall put forward opinions and report them to the director's office meeting for discussion. Qualified medical institutions shall be issued with a Practice License, and unqualified applicants shall be notified in writing. Within 45 days from the date of accepting the application.
