(e) the completion of the construction project, acceptance of documents (limited to new construction, alteration, expansion of radiological treatment of construction projects).
(E) radiation diagnosis and treatment construction project completion and acceptance of documents (limited to new construction, alteration, expansion of radiation diagnosis and treatment construction projects);
(F) radiation diagnosis and treatment equipment within the last year, radiation protection, performance testing reports (except for new construction, alteration, expansion of radiation diagnosis and treatment construction projects);
(VII) continuation of the original license to submit the original radiation diagnosis and treatment license, a copy of the original.
Legal basis: "Business Registration Regulations" Article 5 Application for licensed business projects, the applicant shall, in accordance with the laws, administrative regulations, the State Council decision to the approving authority to apply for approval, after approval, with the approval documents, certificates to the business registration authority to apply for registration. If the examination and approval authority has a limitation on the operating period of the licensed business items, the registration authority shall register the operating period, and the enterprise shall engage in business within the operating period approved by the examination and approval authority.
