City health administrative departments in the districts on the basis of planning, according to the principle of integrated planning, the development of the city's medical institutions set up planning. Article 3 district health administrative departments in the municipal health administrative departments announced the acceptance of the period of uniform acceptance of medical institutions to start the application.
Applications for the opening of medical institutions, units or individuals, should apply for the opening of medical institutions in the district health administrative department to submit an application for preparation. Article IV citizens applying for the opening of individual or partnership clinics, shall have the following conditions:
(1) a physician or higher professional and technical qualifications and obtain a "license certificate";
(2) health and can personally preside over the medical work;
(3) to the date of the application, the application date, two years without the occurrence of a second-class or higher medical errors. Article 5 Units or individuals applying for the opening of outpatient departments and clinics shall submit the following materials to the administrative department of health:
(1) the name and address of the proposed medical institution;
(2) the subjects to be opened, medical equipment, and the status of the professional health personnel;
(3) the floor area of the proposed medical institution;
(4) the treatment of sewage and filtration program. Article 6 The Feasibility Analysis Report submitted to the health administrative department by the unit or individual applying for opening a hospital shall include the following contents:?
(1) the name and address of the proposed medical institution;?
(ii) the proposed medical institution service mode, service time and diagnostic and therapeutic subjects;
(iii) the floor area of the proposed medical institution, the establishment of hospital beds;
(iv) the organizational structure of the proposed medical institution, the status of professional health personnel;
(v) the proposed medical institution's instrumentation and equipment equipment;
(vi) the sewage and dirt (f) sewage, sewage treatment program. Article 7 The district health administration department shall submit the application for opening a medical institution and other materials to be provided to the expert evaluation committee of the medical institution for evaluation.
Medical institutions expert review committee, the number of members to participate in the review must be more than one-half of the total number of expert review committee; application must be more than two-thirds of the members present at the meeting to pass the review, in order to be qualified. Article 8 For the applicant qualified by the expert deliberation committee of the medical institution, the district health administration department shall make a decision of approval or disapproval after reviewing and approving the application according to the planning of medical institution establishment.
To be approved by the applicant, the district health administrative department issued by the "approval of the establishment of medical institutions"; disapproved, shall be in writing to explain the reasons and notify the applicant. Article 9 The Approval Letter for the Establishment of Medical Institutions shall be valid for a period of five years from the date of issuance:
(1) five years for Level III hospitals;
(2) three years for Level II hospitals;
(3) one year for Level I hospitals;
(4) one year for outpatient clinics;
(5) half a year for clinics.
Applications for the preparation of hospitals, in the "medical institution preparation approval" within the validity of the preparatory work can not be completed within thirty days prior to the expiration of the validity of the application to the district health administration to apply for an extension of six months to one year, after approval, can be extended for half a year to one year.
No "medical institutions to build approval" or holding an invalid "medical institutions to build approval", shall not engage in the preparatory activities of medical institutions.? Article 10 The unit or individual who obtains the "approval letter for the preparation of medical institutions" shall apply to the district health administration department for registration of practice after the completion of its preparation work. When applying for registration of practice of medical institutions, the applicant shall submit relevant supporting materials in accordance with Article 10 of the Provisions. Article 11 The district health administration department shall, within thirty days after accepting the application for registration of practice of medical institutions, conduct examination and field verification in accordance with the Provisions. The applicant who meets the conditions for practice shall be issued a "license for practice of medical institutions"; the applicant who does not meet the conditions for practice shall be given a time limit to make corrections. Article 12 A medical institution that changes its name, address, principal person in charge, diagnostic and therapeutic subjects, beds, registered capital, etc., shall apply to the district health administration department and submit the following materials:
(1) Application for Change of Registration of Medical Institution;
(2) Reasons and justifications for applying for change of registration;
(3) Other materials that should be submitted as stipulated by the health administration department. (c) Other materials specified by the health administration department to be submitted.
The district health administration department shall make a decision on whether to approve the change within thirty days from the date of accepting the application. However, for the change of the main person in charge, diagnostic and therapeutic subjects, beds, the district health administrative department shall first submit to the expert evaluation committee of the medical institutions, and based on the results of the expert evaluation committee of the medical institutions and medical institutions set up planning to make a decision on whether to approve the change. Article XIII of the health administrative departments of medical institutions to implement the verification system:
(a) hospitals since the date of issuance of the "license to practice in medical institutions" every three years;
(b) outpatient clinics and clinics since the date of issuance of the "license to practice in medical institutions" every one year of verification.
