(4) the organizational structure of the proposed medical institution, the status of professional health personnel;
(5) the proposed medical institution's instruments and equipment;
(6) sewage, dirt treatment program. Article 10 The district health administrative department shall submit the application for opening a medical institution and other materials to be provided to the expert evaluation committee of the medical institution for evaluation.

(1) Application for Change of Registration of Medical Institution;
(2) Reasons and justifications for applying for change of registration;
(3) Other materials that should be submitted as stipulated by the health administration department. (c) Other materials specified by the health administration department to be submitted.
The district health administration department shall make a decision on whether to approve the change within thirty days from the date of accepting the application.
