3. Mechanical equipment exceeds the maintenance period or is overloaded, or measures are not taken due to equipment defects, resulting in casualties;
4. The working environment is unsafe and no measures have been taken, resulting in casualties;
5. The dust and poison prevention and safety facilities of new construction, reconstruction and expansion projects are not designed, constructed, put into production and use at the same time as the main project, causing casualties.
