The medic doesn't have the option of a needle anymore, instead he walks up to the person who has fallen down and holds down the square to perform the first aid, which will be animated, and you can see the medic inserting the needle into the human body. If you're playing as the attacker, the goal is to take each zone in order, and to do that, you must first take control of each of the zone's capture points. Battlefield 1 players should be able to tell the difference between Breakout Mode and Action Mode, as they are separate modes in the title.

In Breakout Mode, each engagement is a life-or-death battle for the attacking team, while the defending team's planning is put to the test.
