How to apply for a medical institution license

(7) Health technicians with titles outside the region must obtain a Physician's Practice Certificate before applying for a Medical Institution Practice License.

Conditions for approval of the license to practice in each medical (individual) medical institution

The unit or individual applying for a license to start a medical institution must apply to the local health bureau and provide a feasibility study, including the following:

(1) the name of the unit applying for, the basic situation, and the applicant's name, age, specialty, curriculum vitae, and practicing qualification certificate, ID card;

(2) general information on the population distribution, economic and social development of the location;

(3) distribution of medical institutions in the location of about 300 meters;

(4) the health status of the population in the area and the prevalence of diseases and the prevalence of diseases;

(5) the name of the proposed medical institution, the location, the building area and the usable area, functions, tasks, service radius;

(6) the name, location, floor area and practical area, functions, tasks, service radius;

For each medical (individual) medical institution license:

(1) application and initial approval:

(2) Registration of Practice: The applicant units and individuals shall provide the following materials to the County Health Bureau within the validity period of the "Approval Letter for Setting Up Medical Institutions":

1) Legal representative or principal person in charge of the ID card, graduation certificate, title certificate, physician's license certificate and photocopies of one original and two copies each, (graduation certificate, title certificate, physician's license certificate, filed by the registration authority)
