To handle the medical (individual) medical institutions license conditions:
(1) in line with the Autonomous Prefecture "medical institutions set up planning".
(2) in line with the Ministry of Health "basic standards for medical institutions".
(3) Having local household registration.
(4) The floor area of the house reaches 40 square meters or more.
(5) Obtaining a state-recognized secondary school education or above.
(6) Obtain the title of physician in the province and region, and engage in clinical staff of the same specialty in the registered medical institution for more than five years.
(7) Health technicians with titles outside the region must obtain a Physician's Practice Certificate before applying for a Medical Institution Practice License.
(8) Currently engaged in clinical activities must be the same as the physician title specialty (single subject).
(9) The registered capital reaches 10,000 yuan or more.
Conditions for approval of the license to practice in each medical (individual) medical institution
The unit or individual applying for a license to start a medical institution must apply to the local health bureau and provide a feasibility study, including the following:
(1) the name of the unit applying for, the basic situation, and the applicant's name, age, specialty, curriculum vitae, and practicing qualification certificate, ID card;
(2) general information on the population distribution, economic and social development of the location;
(3) distribution of medical institutions in the location of about 300 meters;
(4) the health status of the population in the area and the prevalence of diseases and the prevalence of diseases;
(5) the name of the proposed medical institution, the location, the building area and the usable area, functions, tasks, service radius;
(6) the name, location, floor area and practical area, functions, tasks, service radius;
The feasibility study report includes and tasks, service radius;
(6) the mode of service, time, diagnostic and therapeutic subjects, and bed establishment of the proposed medical institution;
(7) the organizational structure and staffing of the proposed medical institution;
(8) the equipment and instruments of the proposed medical institution;

(10) the communication, power supply, water and sewage, fire fighting facilities of the proposed medical institution;
(11) the source of funds, investment mode, total investment, registered capital;
(12) and accompanied by proof of the creditworthiness of the unit applying for the establishment of the unit or the establishment of the person; applying for the establishment of outpatient clinics, clinics, clinics, health centers, infirmaries, health care centers, health stations, villages, health centers (offices) , nursing station and other medical institutions, according to the situation can be appropriately simplified to set up the feasibility study report.
Application for the opening of hospitals, legal persons, may not have a health care profession, but the main responsible person must be a health technician; application for the opening of outpatient clinics, clinics, clinics, health clinics and so on, the legal person must have the qualification of licensed medical practitioners.
Applying for the license of each medical (individual) medical institution: 25 working days for the preliminary examination and 45 working days for the approval.
For each medical (individual) medical institution license:
(1) application and initial approval:
Application to open a medical institution units or individuals should be to the county health bureau to submit a written application, the county health bureau in the receipt of all the declared materials, such as consent to the county health bureau to apply for the opening of a medical institution units or individuals to issue the "set up a medical institution approval," and disagreement should be given a written reply. The county health bureau shall issue the Approval Letter for Establishment of Medical Institution to the unit or individual applying to set up a medical institution if it agrees to do so, and shall give a written reply if it disagrees. (2) Registration of Practice: The applicant units and individuals shall provide the following materials to the County Health Bureau within the validity period of the "Approval Letter for Setting Up Medical Institutions":
1) Legal representative or principal person in charge of the ID card, graduation certificate, title certificate, physician's license certificate and photocopies of one original and two copies each, (graduation certificate, title certificate, physician's license certificate, filed by the registration authority)
