Patients or other healthcare service users who make reservations or whose family members, friends or accompanying persons make reservations on their behalf for medical services in Shanghai should read the following terms and conditions carefully before completing and signing the registration form.
The terms and conditions stated in the registration form are also stated in the medical services quotation, a copy of which will be provided to the patient and the healthcare recipient making the appointment, or to the family member, friend, or accompanying person authorized by the patient and the healthcare recipient to make an appointment for the Shanghai Medical Service.
This document is to be signed by the patient or client who makes the appointment and by the family member, friend or accompanying person who is authorized by the patient or client to make the appointment.
By signing the Terms and Conditions of Shanghai Medical Service Appointment document, the patient, the client, and the family members, friends and accompanying persons authorized by the patient and the client to make an appointment with Shanghai Medical Service, accepts and agrees to all the terms and conditions contained in this document.
The signed Shanghai Medical Services Appointment Terms and Conditions document, together with the invoice for payment, shall be delivered to the Patient, the Client, and the family, friends, and accompanying persons authorized by the Patient and the Client to make an appointment for the Shanghai Medical Services and to enter into a contract with China Medical Tourism Corporation on the basis of the signed Shanghai Medical Services Appointment Terms and Conditions document and payment. The medical service provider shall be solely responsible for all treatment services provided to the patient and the medical service recipient, including but not limited to surgery, internal medicine, health care, cosmetology and all related issues of inquiries, comments and complaints, and China Medical Tourism Corporation shall not be held liable for the same.
Any emergencies, complications or medical claims arising out of the patient's or client's present or past illnesses or from side effects of medicines, drug allergies or lack of adequate medical care before, during or after the treatment, regardless of the nature of such emergencies, complications or medical claims, shall be the sole responsibility of the healthcare provider or treating doctor, and China Medical Tourism Corporation shall not be held responsible for such emergencies, complications or medical claims. China Medical Tourism Corporation shall not be liable for this.
If patients, clients and their authorized family members, friends or accompanying persons have any complaints regarding the services of the medical service provider, they should address them directly to the medical service provider without any recourse to China Medical Tourism Corporation. Upon receipt of your medical history and diagnostic report, China Medical Tourism Corporation will arrange for a medical expert to have a supplementary consultation with you via e-mail. China Medical Tourism recommends that you bring all your questions to the attention of your treating physician during the consultation. Of course the initial consultation is only a supplement to your case and is optional for you and the medical specialist and is not a mandatory program.
China Medical Tourism reminds you that all treatments, transplants, replacements, surgeries, etc. carry a certain degree of risk, and therefore you and your healthcare professional must agree to and accept all the expected outcomes of your surgical treatment.
After the initial consultation and medical evaluation, China Medical Tourism will provide you with a quote for your treatment, which will include only the services listed therein. If there are any unanticipated costs, we will give you a proper and detailed explanation after the in-person consultation and before the subsequent medical examination or diagnostic report is issued, as long as you are in Shanghai, in order to obtain your authorization. Prior to the surgery you will be required to pay the full cost of the treatment through us to the healthcare provider, booking the relevant previously confirmed medical services, including doctor appointments at the healthcare provider, theatre appointments, nursing services, wards, room bookings and other appointments made for you by the relevant medical team. The quotation provided to you will not include insurance premiums and additional medication expenses or any unforeseen medical expenses in case of complexity, etc. Please be sure to discuss this point with your doctor prior to your surgery or take out medical and travel insurance as recommended prior to your medical trip to Shanghai.
International airfare, domestic airfare, Shanghai visas, etc. are not included in the price quoted. After you have agreed to be treated - please pay 25% of the total quoted price in advance (non-refundable)
30 days prior to arrival in Shanghai - please pay the rest of the amount, i.e. 75% of the total quoted price
It is also important to note that, in case of emergency, the full quoted price will be mandatory to make an appointment for prioritized or urgent medical care, which also means that the patient will be booked in advance for the medical services. It is also important to note that in critical cases the full quoted fee is mandatory to book priority or emergency medical services, which means that the patient will have to pay the full fee to the Chinese medical tourism company in advance. Regarding refunds for unused services (paid for but canceled in advance), the amount of the refund will be returned to the payer in the amount corresponding to the cancellation of the medical trip as described above. Please note that refunds may take up to 3-5 weeks as per SHB's operational procedures.
