Where can I get the special equipment operator's certificate? What do I need?

Special equipment operators shall be registered in the nearest place of work or residence for registration, training and examination. \x0d\ shall be implemented by the Special Equipment Safety Supervision Section of the Bureau of Quality and Technical Supervision. \x0d\\x0d\ Application conditions: \x0d\ Article 15 Applicants should apply at the nearest place of work or residence. The applicant shall submit the following application materials to the examination institution when signing up for the operator examination: \x0d\ (1) Application Form for Examination of Special Equipment Operators (see Annex A, in duplicate); \x0d\ (2) Identification certificate (2 copies); \x0d\ (3) photos (recent 2 inches, front, bareheaded, with color on white background, 3 photos); \x0d\ (4) Education certificate (2 copies of graduation certificate); \x0d\ (5) Health certificate (when the assessment outline has special requirements for physical condition, the hospital will issue this year's physical examination report, 1 copy); \x0d\ (6) Safety education and training certificate (1 class meeting the requirements of the assessment outline, provided by the employer or relevant professional training institutions); \x0d\ (7) Practice certificate (1 copy, which meets the requirements of the assessment outline and is consistent with the application items, and is provided by the employer or relevant professional training institutions). \x0d\ Applicants can also fill in the application through the online registration system of the issuing department or the designated examination institution, and attach a scanned copy of the materials required in the preceding paragraph (PDF format or JPG format). \x0d\\x0d\ See: \x0d\ Examination rules for operators of special equipment TSG Z6001-2013 \ x0d \ x0d \ Note: Special operators' certificates are trained and tested in the Safety Supervision Bureau.
