Trial Notice and Policy Interpretation of COVID-19 Antigen Testing Application Scheme

Notice on Trial Implementation of COVID-19 Antigen Detection Application Scheme

In order to guide the scientific and rational application of COVID-19 antigen testing in all localities, standardize the disposal management after antigen testing is positive, and further improve the ability of "early detection", this scheme is formulated.

First, antigen testing is suitable for people.

Persons who have symptoms such as respiratory tract and fever within 5 days and go to primary medical and health institutions for treatment.

Isolated observers, including home isolated observers, close contacts and sub-close contacts, entry isolated observers, closed control area personnel and control area personnel.

Community residents who need antigen self-test

Second, the application of antigen detection in primary medical and health institutions

Primary medical and health institutions should first choose nucleic acid detection when they receive patients with respiratory tract, fever and other symptoms within 5 days. Do not have the ability of nucleic acid detection and antigen detection.

The main conditions to be met. In order to ensure quality and safety and avoid cross-infection between doctors and patients, primary medical and health institutions should meet the following conditions at the same time: 1. Sample collection and testing personnel should undergo biosafety training and technical operation training, and pass the examination. 2. Have corresponding biological safety cabinets and other instruments and equipment, and do a good job in the protection of medical personnel during testing. 3. The detection operation should be carried out in a relatively independent and well-ventilated space; Encourage grassroots medical and health institutions with laboratory conditions to carry out testing business in the laboratory. 4. Establish the reporting and delivery system and workflow of antigen-positive personnel.

Processing of test results.

1. Grass-roots medical and health institutions should immediately report to the disease control department under their jurisdiction, and the emergency center will transport the antigen-positive personnel to the medical institutions with fever clinics for nucleic acid detection in accordance with the Guide for Transportation of Persons Related to the COVID-19 Epidemic.

2. For people with negative antigen test, primary medical and health institutions should give symptomatic treatment, guide them to observe at home, conduct antigen test once a day for five consecutive days from the date of treatment, avoid social activities, wear masks correctly, wash their hands frequently and ventilate frequently. If the antigen result is negative until the symptoms disappear, no other intervention measures can be taken. Once the antigen result is positive, the positive person should immediately report to the local community, and the community should contact the emergency center to transport it to the medical institution with fever clinic for nucleic acid detection according to the transportation guide for the relevant personnel of COVID-19 epidemic; Sampling swabs, sampling tubes and inspection cards used by positive personnel are put into sealed bags and transported to medical institutions for medical waste disposal.

Preparation of test reagents. Grass-roots medical and health institutions equipped with antigen detection reagents can be brought into centralized bidding and procurement, and through centralized bidding at the provincial level, the price of detection reagents can be continuously reduced and the burden of detection costs can be reduced.

Third, apply antigen detection to isolated observers.

Home quarantine observation, close contacts and sub-close contacts, entry quarantine observation, closed control areas and personnel in control areas shall be well organized and managed by the relevant administrative departments. During the isolation observation, nucleic acid detection was carried out according to the requirements of the current prevention and control plan, and antigen self-test was carried out once a day for the first 5 days.

The main conditions to be met. The isolated observer shall meet the following conditions for antigen detection: 1. Establish antigen self-test management process, including antigen detection reagent collection, personnel information check, detection process supervision, result report, etc. 2. Isolated observers should read the instructions carefully, and sample, add samples and interpret the results in a standardized way according to the specified requirements and procedures; Conditional management departments should supervise and manage the testing process to ensure the authenticity and credibility of the testing results. 3. Do a good job in garbage disposal. No matter whether the result is negative or positive, sample cotton swabs, sampling tubes, test cards, etc. After testing, it is put into a sealed bag, and the management personnel can check the medical waste or handle it according to the procedure.

Processing of test results. If the antigen test is positive, it must be confirmed by nucleic acid test immediately.

Preparation of test reagents. The management department that organizes isolated observation is responsible for the procurement, distribution and management of antigen detection reagents.

Four, community residents antigen testing application

Community residents who need self-test can purchase antigen detection reagents for self-test through retail pharmacies, online sales platforms and other channels.

Precautions. In order to ensure the quality of sampling inspection, residents need to read the instructions carefully, and carry out sampling, adding samples and interpreting the results in a standardized manner according to the specified requirements and procedures.

Processing of test results.

1. If the antigen test is positive, residents should immediately report to the local community, regardless of whether there are symptoms such as respiratory tract and fever, and the community should contact the emergency center to transport residents to medical institutions with fever clinics for nucleic acid testing according to the guidelines for transporting personnel related to the COVID-19 epidemic. Sampling swabs, sampling tubes and inspection cards used by positive personnel are put into sealed bags and transported to medical institutions for medical waste disposal.

2 asymptomatic residents with negative antigen test can be closely observed, and antigen test or nucleic acid test can be carried out when necessary; Residents with symptoms are advised to go to medical institutions with fever clinics for nucleic acid testing as soon as possible; If it is inconvenient to see a doctor, you should be isolated at home, avoid going out for activities, and conduct antigen self-test once a day for 5 consecutive days.

Verb (abbreviation of verb) Confirmation of nucleic acid detection

Nucleic acid detection is the basis of diagnosing COVID-19 infection. In the process of nucleic acid detection and confirmation, if the nucleic acid test is positive, no matter whether the antigen test result is positive or negative, corresponding measures should be taken according to the infected patients in COVID-19 or the confirmed patients in COVID-19; If the nucleic acid test is negative, but the antigen test is positive, it is considered that COVID-19 infected people take measures such as centralized isolation, close observation and continuous nucleic acid test.

Training and publicity of intransitive verbs personnel

In the process of applying antigen testing, all localities should do a solid job in training medical staff in primary medical and health institutions, so that they can fully grasp the characteristics and operating points of antigen testing, ensure the accuracy and reliability of test results, and do a good job in publicity and education of antigen testing for patients and residents. When conducting antigen self-test, the public should seriously follow the relevant basic requirements and procedures, and conduct nucleic acid testing to confirm when necessary.

Policy Interpretation of COVID-19 Antigen Testing Application Program

In order to further optimize the detection strategy of COVID-19, according to the needs of epidemic prevention and control, the comprehensive group of joint prevention and control mechanism in the State Council decided to promote the monitoring mode of "antigen screening and nucleic acid diagnosis", adding antigen detection as a supplement on the basis of nucleic acid detection. Recently, the Mechanism Comprehensive Group has formulated and issued the "COVID-19 Antigen Testing Application Plan", which requires all localities to seriously organize and implement it.

The "Program" stipulates the applicable population for antigen testing: First, people who go to primary medical and health institutions and have symptoms such as respiratory tract and fever and appear symptoms within 5 days; Second, isolated observers, including home isolated observers, close contacts and sub-close contacts, entry isolated observers, closed control areas and personnel in control areas; Third, the community residents who need antigen self-test. At the same time, the main conditions that should be met for antigen detection of three groups of people and the channels for obtaining detection reagents were clarified, and the disposal management process after detection was found positive was formulated to promote the connection between antigen detection and nucleic acid detection. In order to facilitate standardized testing in various places, the scheme also includes two annexes, namely, Basic Requirements and Procedures for Antigen Testing in Primary Health Care Institutions in COVID-19 and Basic Requirements and Procedures for Antigen Self-test in COVID-19.

It should be noted that nucleic acid detection is still the basis for the diagnosis of COVID-19 infection, and antigen detection, as an auxiliary means, can be used for screening specific populations, which is conducive to improving the ability of "early detection". Primary medical and health institutions have the ability to detect nucleic acids, which should be the first choice; Those who do not have the ability of nucleic acid detection can carry out antigen detection, and do a good job in training medical personnel and communicating and guiding patients. Isolation observers and community residents should carefully read the instructions and standardize the operation, and report to the relevant departments immediately once the antigen test is positive; If necessary, carry out nucleic acid testing to confirm.
