There are three reasons for disputes between doctors and patients. First, doctors can't stay out of it. Many doctors who lack medical ethics intentionally or unintentionally bring unnecessary accidental casualties to patients. After causing serious consequences, some patients and their families did not know, or chose to settle after knowing. If they can't accept it, it will easily lead to disputes or medical troubles.
The second aspect is the patient. There is nothing strange in society, no matter good or bad, people will get sick, and those who are usually picky or often dissatisfied with others in society are prone to disputes with doctors when they go to see them, even if there are unnecessary disputes sometimes, they will not blame the doctors. With the prejudice and distrust of doctors in society, this situation is more and more.
The third reason is the system. The whole medical system, from drug circulation to diagnosis and treatment, has problems. In recent years, the anti-corruption efforts have been greatly improved. The price increase of drugs for patients is sky-high It is common for doctors to receive red envelopes. Some doctors are almost businessmen. Their main purpose is to make money. If you are not sick, you should be sick, and if you are sick, you should be ... This will inevitably lead to no disputes between doctors and patients.
Doctors used to save lives, and teachers used to teach and educate people. What about now? Relevant regulatory authorities should pay attention to it, and the deep-seated reason behind the sharp increase in the contradiction between doctors and patients is the problem.
How to answer such a question is problematic.
Because answering such an either-or question is different for everyone, which is related to his own medical experience, relatives or friends, and his views on some cases in society. His answer is biased, and it is inevitable that it will be biased or one-sided.
In recent years, the doctor-patient relationship in some parts of China is not so harmonious. What's the problem? China people have thought a lot and discussed a lot about this issue, but basically there is no unified view. I also want to talk about my own views here. The doctor-patient relationship is tense. I think the problem lies not in medical treatment itself, but in ideological and moral level and integrity level.
After decades of reform and opening up, China has achieved all-round and rapid development in economy, society, science and technology. However, with the rapid growth of economic wealth, some mercenary thoughts have corroded the normal body of society, and the members of society lack the minimum integrity, which leads to extreme events from time to time. Sometimes, doctors are benevolent and less concerned with economic factors. No matter what your situation is, you will die without money, and if you have less money, you will not be treated. The patient, everything is wrong, it is the doctor's fault, he doesn't look at his own problems at all, and even puts forward some unreasonable demands, etc., which leads to poor communication between doctors and patients and sharp contradictions.
To solve this problem, we must first start from the ideological and moral level and establish a social credit system.
Secondly, establish and improve various rules and regulations to regulate the behavior of both doctors and patients.
Third, increase the punishment of doctor-patient cases and adjust the doctor-patient relationship.
Medical disputes occur frequently. The main problem is not the doctor or the patient, but the cost of seeing a doctor. If the cost problem can be solved, I think medical disputes can be reduced by at least 90%.
I think this cost problem can be discussed from two aspects: patients and doctors.
1. Patient
What do patients expect most from seeing a doctor? Less money, good curative effect.
Spending more money will put pressure on patients and their families. If there is pressure, their mood will change, and sometimes a small problem may be magnified, causing medical disputes. I once met an old gentleman who had stomach cancer, but his family didn't tell him the actual condition. After the operation, he refused to cooperate with the doctor. Every time the nurse goes to hang water for him, he always finds fault and sometimes even refuses infusion. At first, everyone thought the patient was really unreasonable. Finally, because the infusion indwelling needle is blocked, it is necessary to re-inject the indwelling needle. The old man flew into a rage and insisted that the nurse didn't make a good indwelling needle. Although the doctors and nurses explained it repeatedly, the old man refused to listen, and finally even swore. Later, my family explained the reason to us. This old gentleman is usually frugal. The list of expenses for this hospitalization is sent to him every day. After reading it, he was distressed. If he doesn't cooperate with treatment, he just wants to save water and money. Actually, there is no other meaning. In other words, everyone understands that all this is related to cost.
Whether the curative effect is good or not is sometimes hard to say. No doctor can guarantee that he will take good care of the disease. Especially for surgeons, postoperative complications are inevitable. Once complications occur, the hospitalization time is long and the treatment cost is high. Patients and their families will have ideas, and medical disputes will arise. If all the expenses are not borne by the patient, but paid by the insurance company or the government, then even if there are complications, patients and their families will treat them rationally and wait for the improvement of their condition. As for who is responsible, this can be identified by authoritative organizations, and medical disputes can be greatly reduced.
2. Doctor
In addition to dealing with clinical diagnosis and treatment, the cost problem is also unavoidable. In the current medical environment, if the patient owes money, the doctor may have to rush the fee, which makes both parties unhappy and even turns into a medical dispute.
Every profession is the same, and the quality and level of doctors are also uneven. So sometimes, the fault of medical disputes really lies with doctors. Once this happens, the patient will refuse to pay the hospitalization fee because it is the doctor's fault that leads to the new hospitalization fee. Then, hospitals and doctors will have to pay this fee. Will the doctor have no mood in his heart? Just when the patient was also emotional, a medical dispute broke out. If doctors don't need to worry about expenses, make up for mistakes wholeheartedly and strive for a good result, the probability of medical disputes will be very low.
summary
If the medical reform can solve the cost problem and really achieve good curative effect with less money, I think medical disputes can be greatly reduced.
This problem is really not a simple matter, nor can it be said that it is the cause of doctors or patients. As the saying goes, everything is two-sided. Another important reason is the medical system.
Doctors in China can be said to be the cheapest medical workers in the world, and every doctor is overworked. Like this epidemic, tens of thousands of doctors across the country went to Wuhan, Hubei Province to support the epidemic area regardless of their own safety. Some doctors were infected, and some doctors recovered after falling down and continued to fight in the front line. But sadly, some doctors never got up after falling.
Now that the epidemic is raging, there are still things that hurt doctors. Before that, doctors were often injured or even killed. There is no bed to call for medical care, no separate ward to call for medical care, poor medical attitude to call for medical care, not calling for medical care in a transfer ward, and so on.
Slapping someone outside may not only lose money but also be detained for several days. But when it comes to medical care, you may need to apologize verbally. Some people who hurt medical treatment with knives may be exempted from criminal responsibility according to disputes between doctors and patients. Of course, the injured doctor is now in prison.
Dr. China sees more than 100 people in the clinic every day, and no one dares to drink or even go to the toilet during working hours. When a doctor meets a patient who doesn't understand, it only takes a few minutes to have an argument. Most hospitals hope for reconciliation, and finally doctors can only apologize in turn.
Of course, the problem is not all patients, and some doctors do have medical ethics problems. The attitude towards patients is bad, and even some doctors overtreatment increases the burden on patients. Sometimes doctors don't explain clearly to patients, which leads to misunderstandings between them.
Now the medical system and high-quality medical resources are concentrated in the top three hospitals in big cities. Patients are concentrated in these big hospitals, and the following secondary hospitals are all, and there are few township hospitals. This also caused the top to squeeze in, and there was nothing to do below. The shortage of resources is naturally more likely to cause contradictions.
So there are many reasons for frequent disputes between doctors and patients. I hope that the future medical reform can gradually solve these problems, doctors and patients can understand each other, and doctor-patient disputes will no longer appear.
I am a senior surgeon in the top 100 hospitals in China. I worked in a 3A hospital, the Second Hospital and the First Hospital. I have also worked in medical management for many years. I have contacted all kinds of patients and handled all kinds of medical disputes. Let me tell you something about my experience at work.
1. Sincere communication is the basis for both doctors and patients to build trust and prevent medical disputes.
I wonder if you have noticed an interesting phenomenon: doctors operate on their relatives or familiar friends. Even if the patient's recovery process is not ideal sometimes, or even complications occur, it is rare to hear of medical disputes between the two sides. Why? Because these patients know that the doctor who treated him must really help himself seriously and must adopt the best treatment method. Even if the disease does not recover, it must be the disease itself, not the doctor's problem. And the doctor dares to take the best treatment for the patient, because he won't worry that the patient will complain about him. It is precisely because both sides have enough trust that medical disputes will naturally not occur. Therefore, the trust between doctors and patients is the key to prevent medical disputes. The trust among doctors, relatives and patients has been formed for a long time, even related by blood. Not every doctor and patient has it.
2. The doctor said: What kind of care have I done and what are the benefits for the patients? Why are you still being complained?
From the perspective of human nature, in fact, any medical staff likes to cure patients and make the operation more perfect. And they all help patients with their hearts. Perhaps because of the lack of knowledge and experience, doctors and nurses did not think carefully or made mistakes. This is not what doctors and nurses want, and it must be something they are ashamed of. Because of this, once patients complain about medical staff, medical staff feel particularly wronged and sad. At present, this kind of medical complaint is also the most common medical dispute complaint received by hospitals. Most of these complaints are similar after investigation, that is, doctors and nurses have tried their best to help patients, but they are too busy to communicate with patients seriously, which leads to misunderstandings among patients. Therefore, it is very simple to deal with such medical complaints and disputes, that is, ask the medical staff to explain clearly to the patients patiently and the problem will be solved. Therefore, doctors should not only treat patients, but also treat their hearts. Only in this way can we become a good doctor satisfied by the masses.
3. What causes patients to have medical disputes in the hospital?
Due to the serious lack of domestic medical science knowledge and the complexity of diseases and medical care, patients simply don't understand the real situation of diseases and medical care, such as how complicated and complicated the diseases are? Why are there complications? Why can't "good people" be cured? . Is it safe to treat this disease in this hospital? Can hospital doctors be trusted? Which doctor has a high level? Is the hospital charge reasonable? Is the inspection equipment in the hospital good? And so on, the lack of this information makes patients feel insecure. Over the years, there has been a direct interest relationship between Chinese medicine hospitals and patients in the medical system, which has made patients wonder whether the hospitals will make profits and cheat their consumers. This further increases the crisis of trust between patients and their families. However, the lack of communication between doctors and patients, the pain and life pressure of patients, and the one-sided propaganda of some media have turned this crisis of trust into a contradiction between doctors and patients, leading to the outbreak of medical disputes.
4. How do European and American countries prevent medical disputes?
In Europe and America, doctors and patients sign contracts with insurance companies to buy insurance. Patients buy sickness insurance and doctors buy medical liability insurance. There is no direct interest relationship between doctors and patients, and both sides benefit from insurance companies. In order to maximize their own interests, insurance companies have long established a mature medical credit system through medical big data, and made an accurate, true, objective and fair dynamic evaluation of the value of each contracted doctor. This information can be consulted by patients at any time. Therefore, patients are not worried that doctors will mistreat themselves for their own benefit. So in a word, the key to prevent medical disputes in European and American countries is to have a mature medical credit guarantee system recognized by both doctors and patients.
Winter will pass and spring will come soon.
With the rapid development of China's economy, the national system is gradually improving. At present, our national universal medical insurance system has been improved, and the medical charges for single diseases will soon be rolled out nationwide, and medical treatment in different places will generally land. There will be no direct interest relationship between doctors and patients in the future. Therefore, under this national medical security system comparable to Europe and the United States, even surpassing Europe and the United States, patients no longer have to worry that medical staff are not good to them. Strengthening the implementation of a series of ethnic medical policies, such as grass-roots units, staying in counties due to serious illness, helping the poor with medical counterparts, building collectivized specialties, and building regional ethnic medical centers, can make patients no longer have to travel long distances and feel at ease in their hometown for medical treatment. It can be said that the spring of universal medical care has come for both doctors and patients.
5. How to have a warm winter before spring comes?
Although the national universal medical insurance policy has been improved, single disease charges have been started in more than a dozen cities, and the policy of medical counterpart poverty alleviation has been adhered to for many years, it still takes time to establish this whole national medical security system, which is comparable to or even surpasses Europe and the United States. Before the arrival of medical spring in this country, our hospital should strictly demand medical staff: first, explain the illness clearly; Second, care for patients 24 hours a day; The third is to manage fees according to diseases, strictly control drugs and consumables, and prevent unreasonable fees. In this way, I think both doctors and patients can spend a beautiful and warm winter before spring comes!
My opinion is also 9 1, but the patient's family is 9.
As a doctor's family, many times my wife came home to complain about how uncooperative the patient was, but the doctor still said that there was a patient with inosine 700+ two days ago, who didn't cooperate that day, was rescued in icu the next day and died a few days later. There are too many similar situations, and you often have to comfort your wife.
There are too many marketing numbers in China, so doctors prescribe more drugs and check more on purpose. If you feel ok today, you think the doctor is fooling you on purpose.
It is not excluded that there are doctors with medical ethics in China, and there are many very responsible doctors. At the same time, doctors who only make money by buying medicine, not to mention medical ethics, even the most basic conscience has been replaced by interests. A child with skin allergy is in hospital. After he went in, he took various blood samples for three days, and his urine and stool were tested. I can understand that children endure pain and are very cooperative. As a result, I was hung for a week, hanging for 6 ~ 10 hours every day, and took a lot of medicine. We are all very cooperative. On the fifth day, all the results came out. In the morning, the doctor came to make rounds to ask what questions, saying that the attending doctor came to ask him before he knew, but he couldn't say clearly why! ! ! He also said that he would draw more blood. I asked him, biochemical and blood routine tests were done, and the results came out. I don't know why he will draw blood for any reason. Not only did he not answer, but he also talked about the necessity of diagnosis. After three blood tests, I still want to continue pumping. Which family member is willing? Which family doesn't hurt? There is another indicator. Sodium chloride is 0.2 lower, and 6 packets of salt water are hung continuously until 3 packets are opened on the seventh day. I asked what is the impact of this low value? Why should salt water be hung continuously? The doctor said, "I wish the patient a good appetite." I really want to thank him. The child has always had a good appetite during his stay in hospital. Tell the doctor as soon as you are hospitalized.
In addition, doctors never take the initiative to perceive the causes of various results, and even prescribe drugs without asking.
Isn't this the root cause of medical trouble?
In the past, the medical level was low, and fever and cold were not a serious illness at all. In the private clinic in the town, it will take half a day at most. Now go to the hospital regardless of the willy-nilly, draw blood first, then the doctor will receive the patient, have a casual look, prescribe medicine, and two bags of medicine, including intravenous drip, cost more than 300 yuan. Before leaving the consulting room, the doctor said, "Go back to take medicine first, but I'm not ready to come back." This is a magical doctor. People know they will come back before they leave. After taking the prescribed medicine, the fever dropped a little in the morning, but not much in the afternoon. Then I quickly went to the hospital to see a doctor, and I only looked at it a few times before prescribing medicine. In this way, more than 500 pills finally cured the fever. Medical science and technology are developed, but the disease is difficult to treat. Is the efficacy of drugs declining or the doctor's conscience buried?
I use a practical example to let everyone analyze and judge. Last year, because my wife was often angry, I took her to a township hospital to see a doctor. This seemingly steady doctor first wrote a blood test list. After the test paper came out, I asked the pharmacist Is there a problem? The chemist simply replied, no problem is normal. I went to the outpatient doctor with the paper. The doctor looked at the test sheet and asked my wife in surprise, are you menopausal? Why is there something wrong with red blood cells? Let's have a cT examination! My wife couldn't hear clearly, but I heard clearly and almost didn't laugh. Out of respect for the doctor, I had no objection, so I asked the doctor to write a checklist. I took my wife out of the clinic and told her what was going on with a smile. My wife said that the doctor might have a brain problem! The registration form clearly says that my wife is 69 years old! If a doctor sees what I said, can you give me a clear explanation?
Both doctors and patients have problems. First, individual doctors have no patience to explain the causes and precautions of patients, and they can't feel a trace of warmth when they look cold. This is the first inducement to make patients feel uncomfortable. Second, individual patients have great pressure in life and work and are in a bad mood. If their family members are sick and have no time to accompany them, it hurts to watch them. There are other reasons for being in a bad mood and having nowhere to release it. It is suggested that medical service institutions can understand the minds of patients and their families, provide psychological course training for medical staff, and truly let patients and their families think.
